PCOS: inositol, spearmint and what the trials show

Polycystic ovary syndrome affects about 1 in 10 women of reproductive age and shows up as irregular periods, excess hair, acne and insulin resistance. Inositol has the best supplement evidence; spearmint tea is a small, cheap extra. Neither replaces a diagnosis or medical care.

Quick answer

The quick answer

  • In trials, myo-inositol made regular periods 1.79 times as likely as placebo, and lowered testosterone.
  • Spearmint tea lowered free testosterone in small trials; visible hair changes were not confirmed.
  • Weight loss of 5–10% (when relevant), strength training and a diet that steadies blood sugar have strong evidence.
  • PCOS raises long-term risks for diabetes and heart disease; regular checks matter.

Story

A story you might recognize

Composite story built from common reader accounts; details changed. Not a testimonial.

She is 27, with periods every 45–60 days and stubborn chin hair. After a diagnosis of PCOS, she wanted to try lifestyle changes and supplements before anything else, and her doctor agreed with a check-in at six months.

She started myo-inositol twice a day, spearmint tea and strength training three times a week. By month five her cycles were coming every 32–35 days. The chin hair was slower to change, and she kept her follow-up appointment.

The plan

The plan

  1. Confirm the diagnosis. PCOS is diagnosed by cycle history, signs of excess androgens and sometimes ultrasound, after ruling out thyroid and prolactin problems.
  2. Movement and food. Strength training and a diet built on protein, fiber and fewer refined carbohydrates support insulin sensitivity.
  3. Try myo-inositol. Split into two doses; judge it over 3–6 months.
  4. Optional: spearmint tea. Twice a day.
  5. Check in at 6 months. Review cycles, skin and blood tests with your clinician.

One supplement with solid trial data and one inexpensive tea, alongside lifestyle and medical care. Evidence grade: A strong · B moderate · C limited · D preliminary.

  1. 01

    Myo-inositol

Supports more regular cycles and insulin sensitivity in PCOS.

What the research shows: A 2023 meta-analysis (1,691 women) found regular periods were 1.79 times as likely with inositol as with placebo, with falls in testosterone and androstenedione. Effects on insulin resistance were smaller and less certain. (PMID 36703143, PMID 29042448)

    • Generally well tolerated; nausea and loose stools at higher amounts.
    • If you take diabetes medicines, monitor blood sugar when starting.
    • If you are trying to conceive or having fertility treatment, tell your clinician.

    In the studies

    • Trials gave mostly 1,000–4,000 mg of myo-inositol a day, with 4,000 mg (in two doses) used most often.

  2. 02Optional

    Spearmint tea

Optional, cheap and pleasant. Small trials found lower free testosterone; visible hair changes were not confirmed.

What the research shows: In 42 women with PCOS, spearmint tea lowered free and total testosterone over 30 days. Women rated their excess hair as improved, but clinicians measuring it with a standard scale did not see a significant change in that short time. (PMID 19585478)

    • Generally safe as a tea. Large amounts may worsen reflux.

    In the studies

    • The PCOS trial gave one cup of spearmint tea twice a day for 30 days.

Left out

What we left out, and why

Berberine for PCOS
Small trials exist, but EFSA’s January 2026 draft opinion could not set a safe intake for berberine, and it must not be used if you could become pregnant. See our post-meal blood sugar guide.
"PCOS detox" teas
No trials, and some contain laxatives.

Safety

Safety and interactions

Questions

Frequently asked questions

Does inositol help PCOS?

In a 2023 meta-analysis, inositol made regular periods 1.79 times as likely as placebo and lowered testosterone. Most of those trials used myo-inositol.

Does spearmint tea lower testosterone?

In a 30-day trial in 42 women with PCOS, two cups a day lowered free and total testosterone. Clinician-measured hair growth did not change significantly in that time.

Myo-inositol or D-chiro-inositol?

Most trials used myo-inositol, alone or in a 40:1 ratio with D-chiro-inositol. High doses of D-chiro-inositol alone are not recommended.
