Where to start

If poor sleep has lasted months, the treatment with the strongest guideline support is not a supplement. The American College of Physicians recommends CBT-I first for adults with chronic insomnia, and medication only after a shared decision when CBT-I alone has not worked [1].

Supplements sit lower on the evidence ladder. The trials behind magnesium, melatonin and tart cherry are mostly small and short, and the effects they report are measured in minutes. That can still be worth trying, but it is a different scale of benefit from what the labels imply.

Magnesium: modest effects, thin evidence

The only meta-analysis of oral magnesium for insomnia pooled 3 trials in 151 older adults: sleep onset was 17.4 minutes faster than placebo, total sleep time was not significantly different, and the reviewers rated the evidence low to very low quality [2].

Two newer trials tested specific forms in younger adults: magnesium bisglycinate gave a small benefit on insomnia scores [3], and magnesium L-threonate (1 g/day for 21 days) improved some sleep-tracker and questionnaire scores vs placebo [8]. Our magnesium for sleep guide compares them.

Melatonin: small effect, label problems, an unresolved safety question

Melatonin is sold as a dietary supplement in the US. Its average effect in trials is small [4], products often do not contain what the label says [5], and pediatric melatonin ingestions reported to US poison centers rose 530% from 2012 to 2021 [10].
