Hormonal acne: what zinc and spearmint can do

Adult acne along the jaw and chin that flares before periods is common in women in their 20s to 40s. Supplements play a small supporting role at best; topical treatments and prescription options do most of the work. Here is what has trials.

Quick answer

The quick answer

  • People with acne tend to have lower blood zinc, and people given zinc had fewer inflamed spots in a 2020 meta-analysis of small trials.
  • Spearmint tea lowered free testosterone in small trials in women with excess androgens; acne itself was not the main outcome.
  • Sudden severe acne with irregular periods, excess hair or scalp hair thinning can be a sign of PCOS or another hormone condition and needs a clinician.
  • DIM, "liver detox" and seed cycling have no good human trials for acne.

Story

Why this trend took off

Social feeds are full of "hormonal acne" protocols: DIM, liver-detox blends, seed cycling and spearmint. A few have small trials; most have none.

The honest picture is that zinc and spearmint have modest data, and the strongest tools are a dermatologist and treatments like topical retinoids, benzoyl peroxide and, for some women, hormonal options.

The plan

The plan

  1. Step 1Basics first

A gentle cleanser, a non-comedogenic moisturizer and daily sunscreen.

  2. Step 2An evidence-based topical

Benzoyl peroxide or adapalene gel (available over the counter in the US) have far better evidence than any supplement.

  3. Step 3Optional: zinc and spearmint tea

    Judge after 12 weeks.

  4. Step 4See a dermatologist

For scarring, painful cysts, or no improvement after 3 months.

The stack

The hormonal acne add-ons

Two modest options to try alongside proven skin care, not instead of it. Evidence grade: A strong · B moderate · C limited · D preliminary.

  1. 01

    Zinc (gluconate or sulfate)

Modest help with inflammatory acne; people with acne tended to have lower blood zinc.

What the research shows: A 2020 meta-analysis found people with acne had lower blood zinc than controls, and people given zinc had fewer inflamed spots, with side effects similar to comparators. The trials were small and varied, and how zinc compares with standard acne treatments is not established. (PMID 32860489)

    • Adult upper limit: 40 mg/day from all sources. Weeks of zinc at 50 mg/day or more can lower copper.
    • Separate zinc from quinolone or tetracycline antibiotics, which are commonly used for acne.

    In the studies

    • The 2020 meta-analysis pooled oral and topical zinc trials in acne; doses varied and are not given in its abstract. Oral zinc above 40 mg a day exceeds the US upper limit.

A record of what researchers gave, in whom; not a dose for you.

  2. 02Optional

    Spearmint tea

Optional, cheap and pleasant. Small trials found lower free testosterone; visible hair changes were not confirmed.

What the research shows: In 42 women with PCOS, spearmint tea lowered free and total testosterone over 30 days. Women rated their excess hair as improved, but clinicians measuring it with a standard scale did not see a significant change in that short time. (PMID 19585478)

    • Generally safe as a tea. Large amounts may worsen reflux.

    In the studies

    • The PCOS trial gave one cup of spearmint tea twice a day for 30 days.

    Evidencepreliminary (Grade D)

What to look for on the label

Plain spearmint (Mentha spicata) tea bags or loose leaf; not peppermint, and not capsules.

We only name brands we have vetted and work with; a specific pick will appear here once one is approved.

Listed picks above total $19.99 at one-time prices on the brands' own sites; most hold about a month of servings at the label's serving size.

Left out

What we left out, and why

DIM (diindolylmethane)
Marketed for "estrogen balance". No good trials for acne, and it can change hormone metabolism in ways that are not well studied.
Liver detox blends and seed cycling
No human trials for acne.
High-dose biotin
Biotin does not help acne, and high doses can distort lab tests, including thyroid and heart tests.

Safety

Safety and interactions

  • Zinc: stay at or under 40 mg/day from all sources and separate it from tetracycline-type antibiotics.
  • Painful cysts, scarring or acne with irregular periods or excess hair need a clinician.
  • Not medical advice. Check with your clinician or pharmacist before starting supplements if you take prescription medicines, are pregnant, trying to conceive or breastfeeding, or have kidney, liver or thyroid disease.
  • Supplements are not evaluated by the FDA to diagnose, treat, cure or prevent any disease.
