Women's Hair Loss: Minoxidil, Spironolactone, Laser Caps and Supplements, by the Evidence

Topical minoxidil has the best evidence for female pattern hair loss, and low-dose oral minoxidil matched it in a pooled analysis of trials. Spironolactone and laser caps have weaker data, and most hair supplements rest on small, short trials.

Minoxidil has to be used continuously; the drug facts label says hair loss will begin again if you stop, and that it will not work for all women [2].

Prescription options

Laser caps and combs

In two sham-controlled trials of a laser comb in women, terminal hair counts rose by about 20 hairs per cm² over 26 weeks vs about 3 with the sham device [5]. The Cochrane review rated the hair-count evidence low quality and found women did not rate laser combs better than sham [1].

Kiierr's 148 and 272 laser caps were cleared through the FDA 510(k) pathway in September 2018 to promote hair growth in women with Ludwig-Savin I-II androgenetic alopecia [7].

Supplements

A 2023 systematic review found some supplements, including Nutrafol and Viviscal, had the higher-quality evidence among those studied, but trials were mostly small and the authors called for larger trials with active comparators [8].
