PMS and PMDD: calcium, chasteberry and what else has trials

Most women notice some changes before a period; for about 1 in 5 they disrupt daily life, and for a smaller group, PMDD, mood symptoms become severe. Calcium and chasteberry have the best trial data among supplements. Here is the evidence, and when to get help.

Quick answer

The quick answer

  • Track symptoms daily for two months first: a clear pattern in the 1–2 weeks before a period, easing after it starts, is what defines PMS and PMDD.
  • Calcium cut PMS symptoms by 48% vs 30% with placebo in the largest trial.
  • Chasteberry made remission about 2.6 times more likely than placebo in a meta-analysis, though trial quality was mixed.
  • PMDD, with severe mood symptoms, deserves a clinician: SSRIs and other treatments have strong evidence. If you have thoughts of self-harm, call or text 988.

Story

A story you might recognize

Composite story built from common reader accounts; details changed. Not a testimonial.

She is 31. For about a week before her period she is snappy, bloated and craving sugar, and twice a year she calls in sick with cramps. She assumed it was just how it is.

She tracked her symptoms for two months, which showed a clear pattern. She started calcium with meals and, after a month, added chasteberry. By the third month the irritability and bloating were noticeably milder. When one month brought dark thoughts, she booked an appointment rather than waiting it out.

In depth

PMS or PMDD?

PMS covers physical and emotional symptoms that start after ovulation and ease within a few days of bleeding. PMDD is a more severe, mood-dominant form: marked irritability, depression, anxiety or feeling out of control that interferes with work or relationships. It affects about 3–8% of women and is a recognized diagnosis with effective treatments.

The plan

The plan

  1. Track for two months. Use an app or a paper chart to rate mood, bloating, breast tenderness, cravings and pain every day.
  2. Start calcium. Split across meals; count dairy and fortified foods.
  3. Chasteberry after one month. Unless you are on hormonal contraception, trying to conceive or pregnant.
  4. Movement and sleep. Regular aerobic exercise and steady sleep help in trials and cost nothing.
  5. Review after three months. If symptoms still disrupt your life, or mood symptoms are severe, see a clinician.

Calcium first, chasteberry second, then optional saffron or B6. Give each at least three months. Evidence grade: A strong · B moderate · C limited · D preliminary.

  1. 01

    Calcium (carbonate or citrate)

The cheapest option with a large trial behind it for mood, bloating, cravings and pain before a period.

What the research shows: In 466 women with moderate to severe PMS, calcium cut total symptom scores by 48% from baseline by the third month, against 30% with placebo. All four symptom groups (mood, water retention, food cravings and pain) improved. One large trial plus smaller ones; graded B. (PMID 9731851)

    • Adult upper limit: 2,500 mg a day from all sources for ages 19–50 (NIH Office of Dietary Supplements). Count the calcium you already get from food too.
    • Calcium carbonate can cause constipation and gas. Separate calcium from thyroid hormone, iron and some antibiotics by at least 4 hours.
    • History of kidney stones: ask first.

    In the studies

    • The 1998 trial gave 1,200 mg of elemental calcium a day as calcium carbonate for three menstrual periods.

  2. 02

    Chasteberry (Vitex agnus-castus) extract

Best for breast tenderness, irritability and mood swings before a period.

What the research shows: A 2019 meta-analysis of double-blind trials found women on chasteberry were about 2.6 times as likely to have their PMS go into remission as women on placebo, but only 3 of 21 trials met reporting standards, and placebo responses were large. Graded B. (PMID 31780016, PMID 23022391)

    • Not for use in pregnancy, while breastfeeding or with hormonal contraception or fertility treatment, because it acts on pituitary hormones. Ask first if you have a hormone-sensitive condition.
    • May interact with dopamine-acting medicines (some antipsychotics and Parkinson’s medicines).
    • Mild stomach upset, headache and acne have been reported.

    In the studies

    • Trials gave a standardized extract once a day; the dose-finding trial gave 8, 20 or 30 mg of the Ze 440 extract a day for three menstrual periods.

  3. 03Optional

    Standardized saffron extract

Optional. Most useful when low mood and irritability lead the picture.

What the research shows: A 2026 meta-analysis found saffron improved PMS symptoms (SMD −0.64) and period pain (SMD −0.51) versus control. The trials were small and mostly from Iran. (PMID 41151539, PMID 18271889)

    • Use standardized extracts at studied amounts only; high doses of saffron are unsafe in pregnancy.
    • Ask first if you take an antidepressant, blood thinners or blood-pressure medicines, or have bipolar disorder.

    In the studies

    • Trials in the 2026 meta-analysis gave 28–90 mg of saffron extract a day; the earliest PMS trial gave 15 mg twice a day for two months.

  4. 04Optional

    Vitamin B6 (pyridoxine)

Optional. Old trials, mixed quality; the amounts used come close to the upper limit.

What the research shows: A 1999 systematic review of 9 trials (940 women) found women were more likely to report overall improvement with B6 (odds ratio 2.32) and depressive symptoms improved, but the trials were small and of poor quality. (PMID 10334745)

    • Adult upper limit: 100 mg a day from all sources (NIH Office of Dietary Supplements). Long-term high-dose B6 can cause nerve damage, with tingling or numbness in the hands and feet; stop and see a clinician if that happens.

    In the studies

    • Trials in the 1999 systematic review gave up to 100 mg of vitamin B6 a day.

A record of what researchers gave, in whom; not a dose for you.

    Evidencelimited (Grade C)

What to look for on the label

Vitamin B6 as pyridoxine hydrochloride or P5P, at a modest amount per tablet. Check every other product you take for added B6.

Left out

What we left out, and why

Evening primrose oil
Popular for breast pain and PMS, but trials have not shown a clear benefit over placebo.
"Hormone balance" blends
Usually small amounts of many herbs, including some that should not be combined with contraception. Single ingredients at studied amounts are easier to judge.

Questions

Frequently asked questions

What supplements help PMS?

Calcium and chasteberry have the strongest trial data; saffron and vitamin B6 have smaller or older trials. Give each at least three months and track your symptoms.

What amount of calcium did the PMS trial use?

The largest trial gave 1,200 mg of elemental calcium a day as calcium carbonate and found symptoms fell 48% versus 30% with placebo by the third month (PMID 9731851).

Can I take chasteberry with birth control?

It is not recommended: chasteberry acts on pituitary hormones and has not been studied with hormonal contraception. It is also not for pregnancy or fertility treatment.

What is the difference between PMS and PMDD?

PMDD is a severe, mood-dominant form of PMS that disrupts work or relationships. It affects about 3–8% of women and has effective treatments, so it is worth seeing a clinician.
