Life on a GLP-1: Muscle, Protein, Hair Shedding and What Happens When You Stop

The questions that come after the prescription, answered from the trials: how much of the weight lost is lean mass, what protein targets guidelines give, why hair sheds and whether it grows back, and how much weight returns when treatment stops.

Key takeaways

  • Roughly 40% of the weight lost on semaglutide and 25% on tirzepatide was lean mass in body-scan substudies, a share similar to diet-based weight loss [1][2].
  • A 2025 joint advisory recommends strength training at least 3 times a week plus 150 minutes of moderate aerobic activity on a GLP-1, and says protein alone is likely inadequate [3].
  • Guidance documents put protein near 1.2 to 1.6 g/kg a day during active weight loss [3][4].
  • Hair shedding on these drugs is linked to weight loss in the labels and is usually telogen effluvium, which is typically reversible [5][6].
  • In withdrawal trials, roughly half to two-thirds of the lost weight returned within about a year of stopping [7][8].

  1. Muscle Loss on Semaglutide vs Tirzepatide: What the Body Scans Show, and What Strength Training AddsIn body-scan substudies, roughly 40% of the weight lost on semaglutide and 25% on tirzepatide was lean mass. Pooled trials found lifestyle programs that added resistance training lost a smaller share as lean.
  2. How Much Protein on a GLP-1? What the Joint Advisory and Expert Consensus SayA 2025 joint advisory and a 2026 expert consensus both land near 1.2 to 1.6 g of protein per kg a day during weight loss, with a lower floor for maintenance. Which body weight to multiply by is still unsettled.
  3. Hair Shedding on a GLP-1: Why It Starts Months Later, Whether It Grows Back, and the Ferritin QuestionShedding linked to weight loss is usually telogen effluvium, which is typically reversible: a 2026 review says regrowth tends to become noticeable within 6 to 12 months once the trigger is removed or treated. Iron is recommended when tests show iron-deficiency anemia; its value for shedding otherwise is debated.
  4. Stopping Wegovy or Zepbound: How Much Weight Comes Back, and What Happens to Blood Pressure and Blood SugarWhen tirzepatide was swapped for placebo in SURMOUNT-4, weight rose 14% over the next year; a year after semaglutide stopped in STEP 1, two-thirds of the loss had returned. A 2026 meta-analysis projects cardiometabolic gains fade within about 1.4 years.

What changes on a GLP-1

Semaglutide (Wegovy) and tirzepatide (Zepbound) cut appetite, and both labels say they lower body weight with greater fat loss than lean loss [9][5]. Eating far less can bring lean-mass loss, nutrient shortfalls and hair shedding; this hub gathers the trial data on each.

Start with muscle loss and strength training and protein targets, then hair shedding and regrowth and what happens when you stop.

What happens when treatment stops

In SURMOUNT-4, the weight of people switched from tirzepatide to placebo rose 14.0% over a year, while those who continued kept losing [7]. A year after semaglutide stopped in STEP 1, participants had regained about two-thirds of their loss [8]. Any change to treatment is a decision to make with your prescriber.

Regulatory and legal notes

  • Compounded semaglutide and tirzepatide are not FDA-approved; the FDA does not review them for safety, effectiveness or quality before they are marketed and says compounded drugs should only be used when an FDA-approved drug cannot meet a patient’s medical need [10].
  • The FDA lists claims that a compounded drug is the same as an FDA-approved drug among its telehealth red flags [10].
  • Wegovy and Zepbound carry a boxed warning about thyroid C-cell tumors seen in rodents; it is unknown whether they cause these tumors in humans [9][5].
