The Wegovy Pill (Oral Semaglutide 25 mg): Trials and FDA Label

What the OASIS 4 trial and the FDA label show for the Wegovy pill: weight change at 64 weeks, the morning fasting rule, the dose steps, side effects and who it is for.

Quick answer

The Wegovy pill is oral semaglutide taken once a day, stepping up from 1.5 mg to a 25 mg maintenance dose. In the 64-week OASIS 4 trial, adults without diabetes lost an average of 13.6% of body weight on the pill versus about 2% on placebo. It must be taken on an empty stomach with up to 4 ounces of water, then nothing else for at least 30 minutes, and it carries the same boxed warning about thyroid C-cell tumors as Wegovy injections.

Only a licensed prescriber can decide whether the Wegovy pill suits you and at what dose. Label facts below are quoted from the FDA-approved prescribing information on DailyMed. Evidence grades: A = several randomized trials or a meta-analysis; B = at least one randomized trial; C = small or pilot human data, expert consensus, or mechanism only; D = animal or cell data, or anecdote.

What the Wegovy Pill Is

The Wegovy pill is semaglutide, the same GLP-1 medicine in Wegovy injections, as a once-daily tablet. Tablets come in 1.5 mg, 4 mg, 9 mg and 25 mg strengths; their inactive ingredients are magnesium stearate and salcaprozate sodium (SNAC) (Wegovy label, DailyMed).

Per the label, Wegovy tablets are approved, with a reduced-calorie diet and more physical activity, for adults: to reduce excess weight and keep it off long term in adults with obesity, or with overweight plus at least one weight-related condition; and to lower the risk of major heart events (cardiovascular death, heart attack or stroke) in adults with established cardiovascular disease and obesity or overweight.

The label says not to combine it with other semaglutide products or any other GLP-1 medicine. Compounded "oral semaglutide" is a different, unapproved product; see compounded vs brand semaglutide.

What the Trial Showed (OASIS 4)

OASIS 4 randomized 307 adults without diabetes (BMI 30+, or 27+ with an obesity-related condition) 2:1 to oral semaglutide 25 mg or placebo once daily, plus lifestyle support, for 64 weeks (Wharton et al., N Engl J Med 2025; PMID: 40934115). Average weight change was −13.6% on the pill versus −2.2% on placebo. Evidence: B (one randomized trial).

The FDA label reports the same trial (Study 7) with a slightly different analysis:

Participants averaged 48 years old, 79% were women, and average starting weight was 106 kg with a BMI of 38 (label). Results in your own case can differ.

How It’s Taken (Per the Label)

• Once each morning on an empty stomach, with water only, up to 4 ounces. Swallow it whole; don’t split, crush or chew it.

• Wait at least 30 minutes before eating, drinking anything else or taking other oral medicines.

• If you miss a dose, skip it and take the next one the following day.

The label’s dose steps, each lasting 30 days, are below. Your prescriber may slow the steps if side effects are hard to tolerate.

DaysDaily tablet
1–301.5 mg
31–604 mg
61–909 mg

Switching between the pill and the weekly Wegovy injection follows specific label instructions, so do it only with your prescriber.

Side Effects and Warnings

Stomach side effects are the most common. In OASIS 4, gastrointestinal side effects occurred in 74.0% on the pill vs 42.2% on placebo (PMID: 40934115). In the label’s account of the trial, 6.8% on the pill and 5.9% on placebo stopped treatment because of side effects.

The label carries a boxed warning: semaglutide caused thyroid C-cell tumors in rodents, and it is not known whether it does in humans. It should not be used by people with a personal or family history of medullary thyroid carcinoma or with MEN 2. Other label warnings include pancreatitis, gallbladder disease, kidney injury from dehydration, low blood sugar when combined with insulin or insulin secretagogues, and aspiration risk during anesthesia, so tell your surgical team you take it. More detail: GLP-1 side effects on the FDA labels.

People with type 2 diabetes: the label notes lower and more variable semaglutide blood levels with the tablets than with the 2.4 mg injection, which may leave some patients under-treated; their clinicians may consider the injection instead.

Pill vs Injection vs Foundayo

The pill trades a weekly injection for a daily fasting routine. Trials of the pill, the injection and Lilly’s Foundayo pill enrolled different people over different lengths of time, so their results can’t be compared head to head. See Foundayo vs the Wegovy pill for prices and food rules, and high-dose oral semaglutide for background.

Medicare Part D enrollees who qualify can get the Wegovy pill through the Medicare GLP-1 Bridge for $50 a month through 2027. Whatever form you take, keep protein up and keep training: see eating on a GLP-1 and protecting muscle on a GLP-1.
