Ozempic vs Wegovy: Is It the Same Drug?

Yes and no. Ozempic and Wegovy contain the same molecule, semaglutide, made by Novo Nordisk, but they are approved for different uses at different doses. Ozempic is a type 2 diabetes medicine dosed up to 2 mg a week; Wegovy is a weight-management medicine dosed at 2.4 mg a week (up to 7.2 mg as Wegovy HD) or as a daily pill. Using Ozempic for weight loss is off-label. Self-pay on Oct 11, 2026, both pens cost $199 a month for a new patient’s first two starter fills, then $349 a month; Ozempic 2 mg is $499 and Wegovy 7.2 mg is $399.

Quick answer

  • Same active ingredient (semaglutide), same maker, same weekly injection schedule and the same boxed warning.
  • Different doses: Ozempic 0.25 mg to start, maintenance 0.5, 1 or 2 mg. Wegovy steps up to 2.4 mg by week 17 and can go to 7.2 mg.
  • Different results for weight because of the dose: Wegovy 2.4 mg averaged 14.9% loss at 68 weeks (STEP 1, no diabetes); Ozempic 1 mg averaged 6.1 kg at 56 weeks in people with type 2 diabetes (SUSTAIN 2), a different population (grade B: one large randomized trial each).
  • Insurance is often the practical difference: Ozempic is billed as a diabetes medicine, while coverage of weight-loss drugs such as Wegovy varies by plan, and a plan may not cover Ozempic prescribed only for weight. Check your formulary.

Which one your prescriber is likely to choose

  • Type 2 diabetes: Ozempic is the labeled choice, and insurance is more likely to cover it.
  • Weight management without diabetes: Wegovy is the labeled choice, at higher doses than Ozempic reaches.
  • Paying cash: the pen prices are the same at the starter and middle doses, so the labeled product for your condition usually makes sense. The Wegovy pill starts lower, at $149 a month.
  • Do not swap pens yourself. The doses and pen strengths differ, and the FDA has received reports of semaglutide dosing errors, mostly with compounded vials.

FAQ

Are Ozempic and Wegovy the same drug?

They contain the same active ingredient, semaglutide, and are made by the same company, Novo Nordisk. They differ in approved uses and doses: Ozempic is for type 2 diabetes up to 2 mg a week, while Wegovy is for weight management at 2.4 mg a week, with a 7.2 mg option and a daily pill.

Can Ozempic be prescribed for weight loss?

A clinician can prescribe Ozempic off-label for weight loss, but it is FDA-approved only for type 2 diabetes. Many insurance plans will not cover it without a diabetes diagnosis, and Wegovy is the semaglutide product approved for weight management.

Is Wegovy stronger than Ozempic?

Wegovy reaches higher doses: 2.4 mg a week, or 7.2 mg as Wegovy HD, compared with Ozempic’s maximum of 2 mg. The molecule is the same, so the difference is the dose, which is why Wegovy’s weight-loss trial results are larger.

Which is cheaper, Ozempic or Wegovy?

Self-pay through NovoCare Pharmacy on Oct 11, 2026, both pens cost $199 for a new patient’s first two starter fills, then $349 a month. Ozempic 2 mg cost $499 a month and Wegovy HD 7.2 mg $399. The Wegovy pill started at $149 a month.

Do Ozempic and Wegovy have the same side effects?

They share the same labeled risks, including nausea, vomiting, diarrhea, pancreatitis, gallbladder disease and the boxed warning about thyroid C-cell tumors. Side effects are generally more common at higher doses and while the dose is being raised.
