The Short Answer
The Part D law excludes "agents when used for weight loss." CMS says that since Part D began it has read that exclusion to cover anti-obesity medicines, so they were coverable only for another accepted use, such as type 2 diabetes or lowering the risk of major heart events in adults with established heart disease (CMS fact sheet, Nov 2024). A proposal to change that interpretation was not finalized in April 2025 (CMS final rule fact sheet).
What changed in 2026 is a separate, temporary program. The Medicare GLP-1 Bridge runs from July 1, 2026 to December 31, 2027 and gives eligible Part D enrollees certain GLP-1 drugs for weight management at a $50 copay (CMS; Medicare.gov).
Which Drugs the Medicare GLP-1 Bridge Covers
| Drug | Form covered by the Bridge |
|---|---|
| Wegovy (semaglutide) | Injection or tablet |
| Zepbound (tirzepatide) | KwikPen only (not single-dose vials or pens) |
| Foundayo (orforglipron) | Tablet |

Who Qualifies
You need Medicare drug coverage. That includes a standalone Part D plan, a Medicare Advantage plan with drug coverage, a Special Needs Plan, an employer/union group waiver plan, or the LI NET program. You must be 18 or older and, when you start the GLP-1, meet one of these (Medicare.gov):
• BMI of 35 or higher.
• BMI of 30 or higher plus heart failure with preserved ejection fraction, uncontrolled high blood pressure, or chronic kidney disease stage 3a or higher.
• BMI of 27 or higher plus prediabetes, a previous heart attack or stroke, or peripheral artery disease with symptoms.
You are not eligible for the Bridge if your Part D plan already covers a GLP-1 for you, or if you have type 2 diabetes, moderate-to-severe sleep apnea or fatty liver disease. Medicare.gov notes that in those three conditions your regular Part D plan might cover a GLP-1 instead.
How to Get It
1. Your clinician sends a prescription for a covered GLP-1 to your pharmacy and, when asked, completes a prior authorization.
2. Your clinician must certify that you are using the drug as part of a lifestyle program focused on diet and exercise.
3. You get a letter confirming coverage under the Bridge. The prior authorization stays valid, including for refills and dose changes, through December 31, 2027 unless you switch to a different GLP-1.
Your pharmacy may ask for your Medicare Number or the last 4 digits of your Social Security Number. Questions go to 1-800-MEDICARE (TTY 1-877-486-2048). Steps from Medicare.gov.
What the $50 Copay Does and Doesn’t Count Toward
You pay $50 to the pharmacy for a one-month supply (28 or 30 days depending on the drug). Because the Bridge runs outside your Part D benefit, the copay (Medicare.gov; CMS):
• does not count toward your Part D deductible or yearly out-of-pocket limit and won’t appear on your Part D Explanation of Benefits;
• can’t be lowered by Extra Help (the low-income subsidy);
• can’t be spread across months with the Medicare Prescription Payment Plan.
Behind the scenes, CMS says participating manufacturers supply Bridge drugs at a net price of $245 per monthly supply (CMS, Information for Part D plans).
If You Don’t Qualify
• Type 2 diabetes, sleep apnea or fatty liver disease: ask your clinician and your Part D plan whether a GLP-1 is covered for that condition. Plan formularies and prior-authorization rules vary.
• Obesity counseling: Part B covers obesity screening and behavioral counseling in primary care if your BMI is 30 or more, at no cost when your provider accepts assignment; you must meet weight-loss requirements for the counseling to continue (Medicare.gov).
• Paying cash: Medicare.gov points to the manufacturer’s assistance programs, state drug-cost help and TrumpRx.gov for price comparisons. Discount-card purchases do not count toward your Medicare deductible or out-of-pocket maximum. For self-pay and savings-card prices, see GLP-1 cost in 2026 and GLP-1 with insurance or Medicare.
What Happens After 2027
CMS extended the Bridge through December 31, 2027 after its BALANCE Model did not launch in Part D for 2027, and says the extra data will help Part D plans prepare for a possible future BALANCE launch (CMS FAQ). Nothing has been announced for 2028, so plan around the end date.
Weight often returns after GLP-1s are stopped: in STEP 4, people switched from semaglutide to placebo regained weight while those who continued kept losing (Rubino et al., JAMA 2021; PMID: 33755728). Evidence: B. See what happens when you stop semaglutide before your coverage window closes.
