Perimenopause brain fog: what helps, and what the supplement trials show

Forgetting words mid-sentence and losing your train of thought are among the most common complaints of the menopause transition. Studies suggest the changes are real but usually temporary. Fixing sleep, hot flashes and mood helps most; creatine and saffron are small extras with some trial data.

Quick answer

The quick answer

  • Memory and word-finding dips are common in perimenopause and tend to improve after the transition.
  • Poor sleep, hot flashes and low mood are the biggest drivers; treating them helps memory most. Hormone therapy and nonhormonal options for hot flashes are worth discussing with a clinician.
  • Creatine improved memory slightly across 16 trials in adults; no trials yet in perimenopausal brain fog specifically.
  • Saffron improved the low-mood and anxiety side of perimenopause in one trial.

Story

A story you might recognize

Composite story built from common reader accounts; details changed. Not a testimonial.

She is 48, a project manager who used to hold every detail in her head. Now she walks into rooms and forgets why, and blanks on colleagues’ names in meetings. She worried it was early dementia.

Her clinician explained that memory dips are common in the transition and usually improve. Night sweats were wrecking her sleep, so they talked about treatment options for those first. She added creatine and kept a notebook. Three months later, with better sleep, the fog had mostly lifted.

The plan

The plan

  1. Fix sleep and night sweats. See our 3 a.m. waking guide, and discuss hot-flash treatment options with your clinician.
  2. Check the basics. Thyroid, B12, iron and vitamin D, plus mood and medicines that affect memory.
  3. Consider creatine. Daily; judge after 8–12 weeks.
  4. Optional: saffron. If low mood or anxiety is part of the picture.
  5. See a clinician. For memory problems that worsen steadily, affect daily tasks or worry the people around you.

The stack

The brain fog stack

Sleep and symptom care first; creatine and optional saffron as small extras. Evidence grade: A strong · B moderate · C limited · D preliminary.

  1. 01

    Creatine monohydrate

Small improvements in memory in trials; the brain uses creatine for energy too.

What the research shows: A 2024 meta-analysis of 16 trials (492 adults) found creatine improved memory (SMD 0.31) and processing speed, with bigger effects in older adults and under stress. No trials yet in perimenopausal brain fog specifically. (PMID 39070254)

    • Raises blood creatinine, which can look like a kidney problem on a lab test; tell your clinician. Ask first if you have kidney disease.
    • May cause water retention and stomach upset at higher amounts.

    In the studies

    • Doses varied across the 16 trials in the 2024 cognition meta-analysis; trials in postmenopausal women found benefits at 5 g a day or more.

  2. 02Optional

    Standardized saffron extract

Optional. Helped the low mood and anxiety side of perimenopause in one trial, not hot flashes.

What the research shows: In 86 perimenopausal women, saffron improved the psychological part of a menopause symptom scale (anxiety and low mood) over 12 weeks; hot flashes did not change. Brain fog was not measured directly. (PMID 34463070)

    • Use standardized extracts at studied amounts only; high doses of saffron are unsafe in pregnancy.
    • Ask first if you take an antidepressant or blood thinners, or have bipolar disorder.

    In the studies

    • The perimenopause trial gave 14 mg of affron twice a day for 12 weeks.

Left out

What we left out, and why

"Nootropic" blends
Usually caffeine plus small amounts of many ingredients. Hard to judge and easy to overdo with coffee.
Ginkgo
Large trials found no benefit for memory in healthy older adults, and it raises bleeding risk with blood thinners.

A 2024 meta-analysis of 16 trials found creatine improved memory slightly in adults, more in older adults and under stress. It has not been tested in perimenopausal brain fog specifically.

How much creatine did the trials use?

Doses varied across the cognition trials; the stack card below lists what researchers gave, including in postmenopausal women.
