Recurrent UTIs and BV: what cranberry and probiotics can do

Repeated bladder infections and bacterial vaginosis are common and frustrating. Cranberry products lowered the chance of another UTI in the Cochrane review; probiotics show promise for BV. Neither treats an active infection, which needs testing and usually antibiotics.

Quick answer

The quick answer

  • Cranberry products lowered symptomatic UTIs by about 30% overall and about 26% in women with recurrent UTIs (Cochrane 2023, moderate certainty).
  • D-mannose did not reduce recurrent UTIs versus placebo in a 2024 UK trial of 583 women.
  • Lactobacillus probiotics improved BV markers in small trials; an investigational vaginal product cut recurrence after antibiotics, but it is not sold over the counter.
  • Fever, back pain, blood in the urine or symptoms in pregnancy need same-day care.

Story

A story you might recognize

Composite story built from common reader accounts; details changed. Not a testimonial.

She is 34 and had four UTIs last year, each needing antibiotics. A friend swore by D-mannose powder; another by cranberry pills.

Her clinician confirmed each infection with a urine culture and talked through prevention options. She started a cranberry product with stated PACs and kept a plan for fast testing if symptoms returned. She had one UTI in the next year.

The plan

The plan

  1. Confirm each infection. A urine culture for UTI, and a clinic test for BV, so the right treatment is used.
  2. Prevention habits. Fluids, not holding urine, urinating after sex, and avoiding douching and scented products.
  3. Consider cranberry. A product with stated PACs, daily.
  4. After BV treatment, optional probiotic. A named Lactobacillus product.
  5. Ask about other options. For three or more UTIs a year, or after menopause, ask about prescription prevention options, including vaginal estrogen, which reduces UTIs after menopause.

  1. 01

    Cranberry (juice or PAC-standardized capsules)

Lowers the chance of another bladder infection in women who get them repeatedly. Not for an active infection.

What the research shows: The 2023 Cochrane review (50 trials, 8,857 people) found cranberry products lowered symptomatic, culture-confirmed UTIs by about 30% overall and about 26% in women with recurrent UTIs (moderate certainty). There was little or no benefit in care-home residents, in pregnancy or with bladder-emptying problems. (PMID 37947276)

    • An active UTI needs testing and usually antibiotics; cranberry does not clear an infection. Fever, back pain or blood in the urine needs same-day care.
    • Cranberry may affect warfarin; ask first if you take it.

    In the studies

    • Trials in the 2023 Cochrane review used cranberry juice, tablets or capsules daily for months; products and amounts varied.

What to look for on the label

Unsweetened cranberry juice or capsules that state proanthocyanidins (PACs) per serving. Sugary cranberry cocktail is mostly sugar.

  2. 02Optional

Lactobacillus probiotic (L. crispatus or L. reuteri strains)

Optional. May support a healthier vaginal microbiome after BV is treated.

What the research shows: In 182 women, oral and vaginal L. crispatus capsules improved BV markers and symptoms. A larger NEJM trial of an investigational vaginal L. crispatus product given after antibiotics cut BV recurrence from 45% to 30%, but that product is not sold over the counter. (PMID 36856121, PMID 32402161)

    • BV needs diagnosis and usually antibiotics; probiotics are an add-on, not a substitute. Unusual discharge, pelvic pain or fever needs a clinician.
    • Severely immunocompromised people should ask before taking probiotics.

    In the studies

    • In the 2023 trial, women took oral or vaginal L. crispatus-containing capsules daily; CFU was not stated in the abstract.

Left out

What we left out, and why

D-mannose
A 2024 UK trial in 583 women (PMID 38587819) found 2 g a day did not reduce recurrent UTIs (51% vs 56% with placebo, not significant). Earlier small studies were more positive, but pooled trials are inconclusive.
Boric acid without a diagnosis
Vaginal boric acid is used for some recurrent infections under medical guidance, but it is toxic if swallowed and not for use in pregnancy. Get a diagnosis first.
