The short answer
From 15 sourcesBifidobacterium longum effects are strain-specific and modest. Strain 35624 eased IBS symptoms in 362 women at one dose only, and missed its main goal in 302 people with bloating. BB536 and strain 1714 trials missed their primary outcomes but improved secondary ones, such as stool frequency, early sleep quality and vitality.

Key facts
- Strain matters
- Health Canada requires the strain designation on probiotic labels; NCCIH notes one strain's effects do not carry over to others12
- IBS (35624)
- 100 million CFU beat placebo on pain and bloating in 362 women, but 1 million and 10 billion CFU did not3
- Constipation in older adults (BB536)
- Primary score missed (P = 0.074); stool frequency improved (P = 0.008) in 80 adults with a mean age of 77.95
- Sleep and mood (1714)
- Primary outcomes missed in 89 and 168 adults; sleep quality and vitality improved at secondary time points78
- Blood sugar (BL21)
- Added to metformin for 12 weeks, BL21 lowered HbA1c more than placebo (p = 0.026) in 79 adults with type 2 diabetes14
What it is
Bifidobacterium longum is a gut bacterium sold as a probiotic; its effects depend on the strain, such as 35624, BB536, 1714 or NCC3001.
How it works
ProposedB. longum is a normal gut resident. Proposed effects run through the gut-brain axis: strain NCC3001 reduced brain responses to negative emotional stimuli on fMRI and strain 1714 altered resting brain oscillations on magnetoencephalography, without changing gut microbiota profiles or inflammatory markers in the NCC3001 trial.911

Missed primaries, positive extras
35624 alone did not hold up in pooled data
A 2017 meta-analysis found that single-strain B. infantis 35624 did not improve abdominal pain, bloating or bowel satisfaction across 3 trials; only multi-strain products containing it did.13

Who should be careful
4 groups- Immune-compromised conditions
- Health Canada's probiotic monograph: do not use if you have an immune-compromised condition (for example AIDS, lymphoma, or long-term corticosteroid treatment).1
- Fever or severe gut symptoms
- Health Canada: ask a health care practitioner before use if you have fever, vomiting, bloody diarrhoea or severe abdominal pain, and stop if digestive upset occurs, worsens or lasts beyond 3 days.1
- Premature infants and seriously ill people
- NCCIH reports severe or fatal infections in premature infants given probiotics, and higher risk in people with severe illness.2
- Pregnancy and breastfeeding
- LactMed has no probiotic entry. In a 184-woman randomized trial, strain NCC3001 taken from about 30 weeks of pregnancy (or from birth) until 12 weeks after delivery did not change anxiety or depression scores; the abstract does not report safety outcomes.10

Common questions
5 answersDoes Bifidobacterium longum help IBS?
Can B. longum help with constipation?
BB536 at 50 billion CFU a day for 4 weeks increased stool frequency in older adults, but the main constipation score did not differ significantly from placebo (P = 0.074).5
Does B. longum 1714 help sleep or mood?
Can B. longum be taken with antibiotics?
Health Canada's probiotic monograph says bacterial probiotics, B. longum included, should be taken at least 2–3 hours before or after antibiotics. It also says not to use probiotics with an immune-compromised condition.1
