Spironolactone for Female Pattern Hair Loss: What the Evidence Shows

What spironolactone is
Spironolactone is an aldosterone antagonist approved for heart failure, add-on treatment of high blood pressure and other conditions; hair loss is not an approved use [1]. Dermatologists use it off-label for hair loss because of its anti-androgen effect [2][3][4].
What the studies show
A 2023 meta-analysis pooling randomized and observational studies reported improvement in 56.6% of women, higher with combination therapy (65.8%) than with spironolactone alone (43.2%), but heterogeneity was significant and 37.8% saw no or only modest improvement [2].

Side effects and monitoring
In the 2023 meta-analysis, reported side effects included scalp itching or scurf (18.9%), menstrual changes (11.9%) and facial hair growth (6.9%) [2]. The label warns of high potassium and says to check potassium within a week of starting and regularly after [1].
Based on animal data, the label says spironolactone may affect sex differentiation of a male fetus, so it is not a drug to take while trying to conceive or pregnant without discussing it with a clinician [1].
The cancer warning, in context
The label notes spironolactone caused tumors in long-term rat feeding studies [1]. A 2022 meta-analysis of 7 studies with 4.5 million people found no significant association with breast, ovarian, bladder, kidney or stomach cancer, and a lower risk of prostate cancer, though the certainty of evidence was low to very low [4].
