Minoxidil: Topical vs Oral, for Men and Women, by the Trials and the Labels

Minoxidil foam and solution are sold over the counter to regrow scalp hair in men and women. Low-dose pills are popular but off-label. What the trials show, the side effects, pregnancy, and why it must be kept away from cats.

Key takeaways

5 points
  • Topical minoxidil is sold over the counter for hereditary hair loss on the scalp; the tablets are approved only for severe high blood pressure, so pills for hair loss are off-label567.
  • In men, 5% solution beat 2% and placebo over 48 weeks1; in women, topical minoxidil roughly doubled the odds of moderate to marked regrowth, and 2% and 5% performed alike2.
  • Across 4 randomized trials, oral and topical minoxidil gave similar hair density; unwanted hair growth was about twice as common with the pills3.
  • The topical labels say minoxidil may be harmful if used when pregnant or breast-feeding, and to keep it out of reach of children and pets56.
  • Cats are especially at risk: in poison-center records, 8 of 62 cats that fell ill after an owner's own minoxidil use died9.

What minoxidil is

Minoxidil widens blood vessels. It is also sold as tablets for blood pressure, and the tablets' label limits them to hypertension that has not responded to a diuretic plus two other blood-pressure drugs7. The same label states that using minoxidil tablets to promote hair growth is not an approved indication7.

As a scalp treatment, minoxidil 5% foam is sold over the counter: twice a day on the label for men and once a day on the label for women56. The women's label says continued use is needed to keep any regrowth6.

Topical minoxidil: what the trials show

In a 48-week trial of 393 men, 5% solution beat both 2% and placebo on hair counts, with more itching and local irritation at 5%1. In the Cochrane review of female pattern hair loss, topical minoxidil increased the chance of moderate to marked regrowth, and four trials comparing 2% with 5% in women found no difference in regrowth or side effects2.

The men's label says results may appear at 2 months and some men need at least 4 months5; the women's label says 3 months, and some women need at least 66.

Low-dose oral minoxidil

A 2025 meta-analysis of 4 randomized trials (279 people) found no difference between oral and topical minoxidil in hair density or diameter, while unwanted hair growth was about twice as common with the oral form3. A 2022 network meta-analysis of trials in men ranked 5 mg oral minoxidil first for terminal hair gain at 24 weeks; by 48 weeks, 1 mg finasteride led8.

Because hair growth is not an approved use of the tablets, there is no FDA-labeled hair-loss dose, and oral minoxidil needs a prescriber7.

Side effects and heart safety

In 1,404 people taking low-dose oral minoxidil, unwanted facial or body hair affected 15.1%, lightheadedness 1.7%, fluid retention 1.3% and a fast heartbeat 0.9%, with no life-threatening events; the study was retrospective and had no control group4. At blood-pressure doses, the tablets' boxed warning covers pericardial effusion, occasionally progressing to tamponade, and worsening angina7.

The topical labels say to stop and ask a doctor for chest pain, a rapid heartbeat, faintness or dizziness, sudden weight gain, swollen hands or feet, scalp irritation or unwanted facial hair56. In more depth: oral minoxidil and your heart and minoxidil side effects.

Pregnancy and breastfeeding

Both topical labels warn that minoxidil may be harmful if used when pregnant or breast-feeding, and the women's label says not to use it for hair loss associated with childbirth56. The tablets' label reports excess hair growth in newborns after exposure during pregnancy and one report of minoxidil passing into breast milk7.

Keep it away from cats and dogs

The labels say to keep minoxidil out of reach of children and pets56. In 211 dog and cat exposures reported to the ASPCA poison center, signs appeared even after drops or licks, and 8 of 62 cats that fell ill after an owner's own minoxidil use died9. How exposure happens and how to prevent it: minoxidil and pets.

Guides for men and for women

Label directions and the trial evidence differ by sex, so each has its own guide: minoxidil for men and minoxidil for women. The men's hair loss hub and the women's hair loss hub cover finasteride, spironolactone, laser caps and supplements.

Common pairings: for men, minoxidil with finasteride (finasteride and minoxidil together); for women, minoxidil with or without spironolactone (women's combinations). The finasteride page covers that drug's warnings.
