Where they differ
From the rowsThe best-graded row, grade D, is an observational study of 2,149 people (2023) and found a worse outcome.
The 2 rows that found a worse outcome were larger: a median of 1,085 people, against 24 people for the row that found an improvement.
The 2 rows that found a worse outcome are newer (2023 to 2024) than the row that found an improvement (2014).
The rows
1 improved · 2 worse- DGrade DHeart attack, stroke or death with higher blood erythritol · Worse
Observational · n 2,149 · 2023 · Stable US patients having elective cardiac evaluation (validation cohort; a European cohort of 833 and a discovery cohort of 1,157 also studied)
Not applicable (blood level); 8 volunteers also drank an erythritol drink · 3-year follow-up
Top vs bottom quarter of blood erythritol: adjusted hazard ratio 1.80 (95% CI 1.18 to 2.77) in the US cohort and 2.21 (1.20 to 4.07) in the European cohort. In 8 volunteers, an erythritol drink kept blood levels raised for more than 2 days, above levels that increased platelet reactivity in the lab.
- DGrade DPlatelet reactivity after a 30 g drink (vs glucose) · Worse
Open-label trial · n 20 · 2024 · Healthy volunteers, 10 per group
30 g single dose · Single intervention
Blood erythritol rose more than 1,000-fold (median 6,480 vs 3.75 µmol/L) and stimulated platelet aggregation increased in every subject, with more release of serotonin and CXCL4 from platelets. Glucose caused no such increase.
- DGrade DBlood vessel function in type 2 diabetes · Improved
Open-label trial · n 24 · 2014 · Adults with type 2 diabetes (single-arm pilot)
36 g/day, plus a single 24 g dose at the first and last visits · 4 weeks
A single dose improved fingertip endothelial function (0.52 to 0.87 au, P = 0.005). After 4 weeks, central pulse pressure fell from 47 to 41 mm Hg (P = 0.02) and pulse wave velocity tended to fall (P = 0.06).
