- One of the most-discussed clinics; threads center on onboarding, separately billed labs and its partner pharmacy, with both endorsements and service complaints. (Reddit, low weight, not scored)
- ⚠ Price breakdown: $99.99/mo; the first order covers 2.5 months ($249.97) plus the $129 TRT lab order, about $379 to start.
- ⚠ The 12-month figure counts the first order ($249.97 for 2.5 months + $129 labs) as month one and $99.99 a month after; TRT Nation also asks for a follow-up lab order (about $129) after 10 weeks, which roughly offsets that rounding.
- ⚠ Outside labs are accepted instead of the $129 order.
- ⚠ California pricing may vary; refills are requested, not automatic.

How we score
Re-weight the rubric
Every provider gets the same five pillar scores. Move the sliders to weigh what matters to you; “Your pick” updates, our medals do not.
Known names we don’t rank (yet)
Readers ask about these providers, so here is where they stand. Each row shows the price the provider publishes and the fact that keeps it out of our ranking. We will re-check them; a provider that fixes the issue can be ranked.
| Provider | Published price | Why it isn’t ranked | Community signal |
|---|---|---|---|
| Marek Health | $299 to start + $450 minimum lab panel; medication quoted after consult | Passed our trust checks, but publishes no ongoing monthly price, so it cannot be compared on cost. | Recent mentions sit mostly in comparison threads, where cost is the recurring complaint. |
| PeterMD | Injectable TRT $139/mo month to month, or $79/mo on a 12-month plan ($948 upfront) | Trustpilot removed its profile for violating Trustpilot’s guidelines. | Often used as a price reference in comparison threads; shipping and pharmacy complaints recur. |
| Maximus | Injectable testosterone from $99.99/mo | FDA warning letter 730095 to Maximus Health Inc., Jun 8 2026. | Discussed mainly for oral testosterone and enclomiphene; a widely commented thread disputes its CEO’s health claims. |
| Defy Medical | Average under $175/mo in year one, about $125/mo after (its own estimate) | Offers anabolic steroids beyond testosterone (oxandrolone, nandrolone, stanozolol); no LegitScript seal; two of its own FAQs give different TRT costs (under $175 vs about $250/mo in year one). | Heavily comparison-shopped; threads focus on its broader menu and non-standard protocols. |
| Talon Wellness | As low as $65/mo (its estimate; labs and consult included) | Its men’s menu lists nandrolone, oxandrolone, stanozolol, HGH and IGF-1 LR3; no clinicians are named. | Frequently discussed for its pricing and a broader menu that includes non-testosterone compounds. |
| Henry Meds | From $129/mo (compounded), described as all-inclusive | BBB rating F, not accredited, with 234 complaints in 3 years. | Recent threads are mostly about leaving, citing cancellation difficulty and prescription errors. |
Prices and facts checked Oct 8, 2026 on each provider’s own site and public records (FDA warning letters, BBB, Trustpilot, LegitScript). Community signal is Reddit, low weight, not scored.
Legend: medals, badges and icons
- Gold: highest score on our default weights in that service. One medal per provider per service, and only where at least two providers compete. Gold is “Our pick”.
- Silver: second-highest score on our default weights in that service. One medal per provider per service, and only where at least two providers compete. Gold is “Our pick”.
- Bronze: third-highest score on our default weights in that service. One medal per provider per service, and only where at least two providers compete. Gold is “Our pick”.
- Your pick: the top score after you re-weight the rubric. It can differ from ours, and that is the point.
- Best value: Lowest effective monthly cost for the same medication or service.
- Lowest upfront: Smallest first charge to start, in this service, among full plans that include the medication.
- Labs included: Lab work is part of the published price.
- Brand-name option: The provider also offers a brand-name route (priced separately).
- Best for women: Highest-scoring provider built for women in this service.
- Icons: flask = labs, pill = medication, stethoscope = clinician visits, truck = shipping; struck-through means not in the price.
- Score ring: 0–100 weighted score. Green 80+, orange 65–79, amber below 65.
How we score
Five pillars, each scored 0–100 from facts on the provider’s own pages, then weighted: Price and value 30%, What's included 20%, Trust and compliance 20%, Customer reviews 15%, Offer strength and flexibility 15%.
- Price and value (30%): Effective monthly cost over 12 months on the cheapest published cash plan, including intro prices, renewal credits and 28-day billing. The cheapest option for the same medication scores 100; others score cheapest ÷ theirs × 100. A visits-only or membership-only plan, where the medication is bought separately, scores 40: its total monthly cost is not published, so it neither wins on price nor sets the benchmark.
- What's included (20%): Labs 30, medication in the price 30, clinician visits and follow-ups 20, shipping 20, as the provider’s own page states.
- Trust and compliance (20%): LegitScript seal on the site 40; a named prescriber group or named physicians 30 (licensed but unnamed 15); a 503A/503B pharmacy stated on its pages 30 (US-licensed, type not stated 15; not stated 5).
- Customer reviews (15%): Trustpilot star rating, shrunk toward 3.5 stars until a provider has enough reviews (rating × count + 3.5 × 50, ÷ count + 50), mapped 1–5 stars to 0–100. No verifiable rating scores 40, not zero and not average.
- Offer strength and flexibility (15%): A published first-order discount or intro price 40, a cancel-anytime or month-to-month plan 30, HSA/FSA 15, full refund if a clinician declines you 15.
Effective monthly cost is the cash you would pay for 12 months on the cheapest published plan, divided by 12: the first charge (including any published intro price or first-order discount), then the regular or renewal price for the rest of the year, with 28-day plans counted as 13 charges. Prepaid plans count their full upfront price. “To start” is the smallest first charge on any plan.
Reviews and community signal. Trustpilot ratings feed the reviews pillar, but a Trustpilot rating is not proof of quality: profiles can be unclaimed, cover a parent brand, or be removed, and we note when they are. The “Community signal” line on each provider is a paraphrased summary of Reddit threads from the past 12 months. Reddit is a low-weight, qualitative input: it never changes a score or a medal, it is not a representative sample, and it is not medical advice.
Who is listed: providers that pass our vetting (licensed prescribers, a licensed pharmacy, honest compounded-drug labelling, a clean regulatory record, no research-use-only products, no anabolic steroids beyond testosterone) and publish a price we can compare. Most run an affiliate program we have joined or applied to; vetted providers without one are ranked the same way and get a plain, untracked link. On the TRT and women’s HRT pages, well-known names we do not rank are listed with the reason, without links. Dataset version 2026-10-08.3, prices verified Oct 8, 2026. Full methodology · Disclosure
Where offerings overlap
- 6 providers sell testosterone therapy: effective cost runs from $123/mo (TRT Nation, injections, 2.5-month first order) to $258/mo (Rex MD). Same molecule; the gap is billing structure, what is bundled and how long you commit.
- Labs are in the price only at MangoRx, Hone Health, Rex MD.
- MangoRx, Hone Health, Male Excel, Rex MD, Fountain TRT publish “starting at” prices; the final price is set at checkout or after intake, so treat those rows as a floor.
FAQ
Does online TRT include lab work?
Some providers include an initial lab test in the monthly price; others bill labs separately. We mark labs as included only when the provider’s own page says so, and flag it when its pages disagree.
Which TRT providers are ranked, and why not others?
Hone Health, TRT Nation, Male Excel, Rex MD, MangoRx and Fountain TRT: providers that sell testosterone therapy today, publish a monthly price and pass our vetting. Fountain TRT runs no affiliate program, so its link is plain and untracked. Marek Health, PeterMD, Maximus, Defy Medical, Talon Wellness and Henry Meds are listed under “Known names we don’t rank (yet)” with their published price and the specific reason, such as an FDA warning letter, anabolic steroids beyond testosterone on the menu, or no published monthly price.
Is compounded testosterone FDA-approved?
No. Compounded testosterone is prepared by a pharmacy for an individual prescription and is not FDA-approved. Hone Health and Male Excel say theirs is compounded; the others leave the formulation to the prescriber. Each row shows the form the provider states.
Is TRT available in every state?
No. Coverage varies by provider and state; we note state limits on each row. Eligibility is confirmed during intake.
Is this medical advice?
No. A licensed clinician decides whether testosterone therapy is appropriate for you and what is prescribed.
