Low Testosterone Symptoms in Men

Fatigue, low mood and a softer middle have many causes. In a study of 3,369 men aged 40 to 79, the symptoms that tracked most closely with low testosterone were sexual: fewer morning erections, low sexual desire and erectile dysfunction. Symptoms alone never diagnose low T. The American Urological Association uses a total testosterone below 300 ng/dL, measured twice on separate early mornings, together with symptoms or signs.

Quick answer

  • Most specific symptoms: poor morning erections, low sexual desire and erectile dysfunction. In the European Male Ageing Study these three were the only symptoms with a syndrome-like link to low testosterone; fatigue, low vigor and depression were linked more loosely.
  • The number: the AUA uses a total testosterone below 300 ng/dL as a reasonable cutoff. The European study defined late-onset hypogonadism as at least three sexual symptoms plus total testosterone under 11 nmol/L (about 320 ng/dL) and low free testosterone.
  • The test rule: two total testosterone measurements on separate days, both early in the morning. Testosterone peaks around 3 to 8 a.m. in men with usual sleep hours, so a single afternoon result can mislead.
  • Test even without symptoms if you have unexplained anemia, bone loss, diabetes, chronic opioid or steroid use, HIV, infertility, pituitary problems or past chemotherapy or testicular radiation (AUA).
  • Cost: Mito Health listed a total testosterone test at Quest for $4.32 (members, $9/mo) or $6.05 (non-members) on Oct 11, 2026, plus any itemized lab fee.

The symptoms, ranked by how closely they track testosterone

The European Male Ageing Study surveyed a random sample of 3,369 men aged 40 to 79 in eight countries and measured morning testosterone by mass spectrometry. Six symptoms were significantly related to testosterone levels: poor morning erections, low sexual desire, erectile dysfunction, inability to do vigorous activity, depression and fatigue. Only the three sexual symptoms had a syndrome-like association, and the more of them a man had, the lower his testosterone tended to be.

The likelihood of the sexual symptoms and limited physical vigor rose as total testosterone fell through roughly 8 to 13 nmol/L (230 to 370 ng/dL). The study’s definition of late-onset hypogonadism required at least three sexual symptoms plus total testosterone below 11 nmol/L and free testosterone below 220 pmol/L.

  • Strongest link: fewer or weaker morning erections, low sexual desire, erectile dysfunction.
  • Weaker link: trouble with vigorous activity (running, lifting heavy objects), low mood, fatigue.
  • Signs a clinician looks for: anemia, bone loss, loss of body hair or muscle, breast tissue growth.

Low T symptoms in young men

The AUA cutoff of 300 ng/dL applies at any adult age, and the guideline lists conditions that justify testing even without symptoms, including infertility, chronic opioid or corticosteroid use and pituitary problems. In younger men, a low result calls for a search for the cause: the AUA recommends measuring luteinizing hormone (LH) in every man with low testosterone, and prolactin when LH is low or low-normal.

Testosterone therapy suppresses sperm production. The AUA says men interested in future fertility should have a reproductive health evaluation before treatment, and the Endocrine Society recommends against starting testosterone in men planning fertility in the near term. Our [enclomiphene vs TRT guide](/blog/enclomiphene-vs-trt) covers the fertility-sparing option some clinics prescribe.

What is a normal testosterone level?

The AUA chose 300 ng/dL as a reasonable cutoff based on mean levels in the best available studies. The Endocrine Society recommends measuring fasting morning total testosterone with an accurate assay, repeating it to confirm, and using free testosterone (by equilibrium dialysis or an accurate formula) when the total is near the lower limit or SHBG is likely abnormal.

If one test is under 300 ng/dL and the repeat is normal, the AUA leaves a third test to the clinician’s judgment; it does not insist on fasting, since the evidence that food lowers testosterone is weak. Use the same lab and the same assay for both tests so the numbers are comparable.

Why an online low-T quiz cannot diagnose you

Symptom questionnaires are everywhere, but the AUA does not recommend validated questionnaires to define who should get testosterone therapy or to track response. Low energy, low mood and weight gain overlap with depression, poor sleep, sleep apnea, alcohol and many medicines. Use symptoms to decide whether to test, and the blood tests to decide what they mean.

What treatment does and does not improve

For men with confirmed low testosterone, the AUA tells clinicians to explain that therapy may improve erectile function, low sex drive, anemia, bone density, lean body mass and depressive symptoms, and that evidence is inconclusive for cognition, diabetes measures, energy, fatigue, lipids and quality of life.

Before starting, the AUA recommends measuring hemoglobin and hematocrit and, in men over 40, a PSA. FAQ

What are the first signs of low testosterone in men?

In the European Male Ageing Study, the symptoms most tied to low testosterone were sexual: fewer morning erections, low sexual desire and erectile dysfunction. Fatigue, low mood and reduced vigor were linked more loosely and have many other causes.

What testosterone level is considered low?

The American Urological Association uses a total testosterone below 300 ng/dL as a reasonable cutoff, confirmed by two early-morning tests on separate days and combined with symptoms or signs.

Can I have low T symptoms with normal testosterone levels?

Yes. Symptoms like low energy, low mood and low desire are non-specific and are often caused by sleep problems, depression, stress, alcohol or medicines. If two morning tests are normal, testosterone therapy is not indicated under the AUA guideline; the cause is worth looking for elsewhere.

What time of day should testosterone be tested?

Early morning. Testosterone peaks around 3 to 8 a.m. in men with usual sleep hours, and the AUA requires two early-morning measurements on separate occasions before diagnosing low testosterone.

How much does a testosterone test cost without insurance?

On Oct 11, 2026, Mito Health listed total testosterone at Quest for $4.32 for members ($9/mo) or $6.05 without membership, and free plus total testosterone for $12.15 or $17.01, plus any itemized lab fee.

Can young men have low testosterone?

Yes. The same 300 ng/dL cutoff applies, and the AUA recommends testing men with infertility, chronic opioid or steroid use, or pituitary problems even without symptoms. A low result in a young man should prompt LH and prolactin testing to find the cause, and a fertility discussion before any treatment.

Does an online low-T quiz tell me if I need TRT?

No. The AUA does not recommend symptom questionnaires to decide who gets testosterone therapy. They can prompt a test, but only two low morning blood tests plus symptoms or signs make the diagnosis.

Will TRT fix my energy and fatigue?

Maybe not. The AUA says evidence is inconclusive on whether testosterone therapy improves energy, fatigue or quality of life; benefits are better supported for erectile function, sex drive, anemia, bone density, lean mass and depressive symptoms.
