Summary

Mucuna seeds contain levodopa, the main drug for Parkinson's disease, so supplements act as an unregulated dose of a prescription medicine. In small Parkinson's trials, mucuna powder matched levodopa for motor response, though half of one daily-use group stopped. US supplements contained 2-241 mg of levodopa per serving, unpredictable from their labels.

Key facts

Safety first: it is levodopa
Mucuna seeds contain levodopa (median 5.3% in dried or roasted seeds); supplements are a hidden, variable dose of a Parkinson's drug[6][7]
Supplement levodopa content
15 of 16 US products contained levodopa, from 2 mg to 241 mg per serving; where extract amounts were listed, actual levodopa was 228% to 2,186% of the estimate[7]
Serious drug interactions
For levodopa medicines, non-selective MAO inhibitors are contraindicated and dopamine-blocking antipsychotics reduce the effect[9]
Parkinson's trials
Single doses of roasted-seed powder matched dispersible levodopa/benserazide for motor response in 18 people; in a 16-week daily trial, 7 of 14 stopped mucuna[1][2]
Misuse risks
Unsupervised use has been associated with dyskinesia, dopamine dysregulation syndrome and psychiatric complications[8]

How it works

EstablishedLevodopa in the seed crosses into the brain and is converted to dopamine. Supplements contain no decarboxylase inhibitor (carbidopa or benserazide), so much of the levodopa is converted outside the brain; in one study, mucuna at a similar levodopa dose gave a 3.5-fold lower blood exposure than levodopa with an inhibitor.[6][7][9]

Medicines

  • Non-selective MAO inhibitorsDrug labele.g. Phenelzine, tranylcypromine, isocarboxazidContraindicated with levodopa medicines; the label requires stopping them at least two weeks before levodopa. Mucuna supplies levodopa.[9]
  • Antipsychotics and other dopamine blockersDrug labele.g. Haloperidol, risperidone, phenothiazines, metoclopramideDopamine D2 blockers can reduce levodopa's effect, and levodopa can worsen psychosis.[9]
  • Prescription levodopa and other Parkinson's medicinesDrug labele.g. Carbidopa/levodopa, MAO-B inhibitors such as selegilineAdds an unmeasured levodopa dose; supplement levodopa can unpredictably complicate Parkinson's management, and selegiline with levodopa can cause severe orthostatic hypotension.[7][9]
  • Blood-pressure-lowering medicinesDrug labele.g. Any antihypertensiveSymptomatic postural hypotension has occurred when levodopa was added to antihypertensives.[9]

Other supplements

  • IronDrug labele.g. Iron salts or multivitamins with ironIron can bind levodopa and lower its absorption.[9]

Who should be careful

Anyone on MAO inhibitors, antipsychotics or Parkinson's drugs
Levodopa interacts with all three; changes to Parkinson's therapy belong with a neurologist.[9][8]
People with psychosis or a history of impulse-control problems
Dopaminergic drugs can cause hallucinations, psychotic-like behaviour and compulsive urges.[9][8]
Pregnancy and breastfeeding
Levodopa crosses the placenta and is found in breast milk; there are no adequate studies in pregnant women.[9]
People with heart rhythm problems or narrow-angle glaucoma
Levodopa medicines carry cautions for arrhythmia after heart attack and are contraindicated in narrow-angle glaucoma.[9]
People with melanoma or a history of it
The levodopa label advises frequent, regular skin checks for melanoma; people with Parkinson's have a 2- to about 6-fold higher melanoma risk, and whether levodopa contributes is unclear.[9]

Talk to your pharmacist or clinician before starting a supplement, especially if you take prescription medicines.

Safety first

Labels are unreliable, and the active ingredient is a prescription drug.

Hidden levodopa
Measured levodopa ranged from 2 to 241 mg per serving; prescription tablets typically contain 50-250 mg.[7]
Dopaminergic side effects
Levodopa can cause dyskinesia, hallucinations, sleepiness and compulsive behaviours.[9]
Expert view
Neurologists advise keeping mucuna use within clinical trials or neurologist-led protocols until more safety data exist.[8]
Seed preparation
Cases of toxicity from improper seed preparation have been reported.[8]

How to read the label

  2. No carbidopaNo supplement tested contained carbidopa or benserazide.[7]

Frequently asked questions

Does mucuna contain L-dopa?

Yes. Seeds contain about 5% levodopa, the main Parkinson's drug.[6]

How much L-dopa is in mucuna supplements?

US products contained 2 to 241 mg per serving, and amounts could not be predicted from the label.[7]

Can mucuna replace Parkinson's medication?

Small trials found similar short-term motor effects, but tolerability with daily use was poor; experts advise neurologist supervision only.[1][2][8]

Is mucuna safe with antidepressants?

Non-selective MAO-inhibitor antidepressants are contraindicated with levodopa medicines.[9]

Does mucuna raise testosterone or fertility?

Uncontrolled studies in infertile Indian men reported better semen measures; we found no placebo-controlled trials.[4]

Is mucuna safe in pregnancy?

Levodopa crosses the placenta and enters breast milk, and there are no adequate studies in pregnancy.[9]
