Magnesium orotate

Also called Mg orotate, magnesium orotate dihydrate, Magnerot. One of 9 forms of magnesium compared on this page.

The short answer

From 9 sources

Magnesium orotate pairs magnesium with orotic acid. One 79-patient trial in severe heart failure reported better one-year survival with high-dose orotate added to standard care, and a Russian meta-analysis of 19 small trials linked it to fewer heart-rhythm and blood-pressure problems. Both have major limits. Each gram supplies only about 66 mg of magnesium.

Key facts

What it is
Magnesium salt of orotic acid; 6.6% elemental magnesium as the dihydrate (7.3% anhydrous)26
Top studied outcomes
Survival and symptoms in severe heart failure (C, one trial); heart-rhythm and blood-pressure problems in pooled Russian trials (C)34
Studied dose range
About 1,900 mg/day of the compound on average in pooled trials; 3,000 mg/day in a fertility pilot; 3,000 to 6,000 mg/day in the heart-failure trial347
RDA / UL
Needs 310 to 420 mg/day overall; 350 mg/day maximum from supplemental magnesium1
Documented drug interactions
Like any magnesium: space from bisphosphonates and some antibiotics; diuretics and long-term PPIs change magnesium levels1
Regulatory safety view
EFSA (2025) could not establish the safety of up to 6,100 mg/day of magnesium orotate dihydrate because of orotic acid9

What it is

Magnesium orotate is magnesium bound to orotic acid; the dihydrate is about 6.6% magnesium by weight, less than most magnesium forms.

How it works

Proposed

The orotate form supplies magnesium, which the body needs for hundreds of enzyme reactions. No human study cited here shows that the orotic acid part adds an effect beyond the magnesium it carries.13

What the human research says

Human trials only, one row per outcome. The form line shows what each trial actually tested. Grades follow our evidence-grading policy.

Grade C for orotate itself. 4 outcomes on this page were tested with orotate: heart-rhythm, blood-pressure and related problems; survival and symptoms in severe heart failure; exercise capacity and extra heartbeats after bypass surgery; sperm quality in idiopathic male infertility (Meta-analysis, n 1,190, 2015). Rows that tested other or mixed forms describe magnesium in general, not orotate. For the overall evidence, see the Magnesium page.

What the numbers add up to

Each point is worked out from the data and the sources numbered on this page, with the arithmetic shown under it. It adds no new data and is not medical advice.

One striking trial, never repeated

The 79-patient MACH trial reported 75.7% vs 51.6% one-year survival, a large effect from a single centre, published as a letter and not replicated in the cited literature.3

How we got this: MACH trial design and publication format

Low magnesium per pill

At about 6.6% magnesium, orotate carries far less magnesium per gram than most forms, so reaching a typical 200 mg dose of magnesium needs about 3 g of the compound.21

How we got this: PubChem molecular weight and ODS dosing context

Studied doses

Doses studied in human trials

NIH ODS updated Jan 6, 2026

Doses studied in human trials ranged from a mean of 1,878 mg a day of the compound over about 4 months in pooled Russian trials, through 3,000 mg a day for 90 days in a male-fertility pilot, to 6,000 mg a day for 1 month followed by 3,000 mg a day for about a year in the heart-failure trial.347

Intake reference: Total magnesium needs are 310 to 420 mg/day for adults; supplemental magnesium, orotate included, should stay within 350 mg/day.1 Food magnesium is excluded from that ceiling. NIH ODS fact sheet (updated January 6, 2026).

Starting from one medicine? The interaction checker rates how serious each pair is, on a curated list of 19 entries for 11 medicines. This cabinet has no severity rating: it lists every interaction row our supplement pages cite.

Not medical advice, and no dosing advice. An empty result means no row we cite names that pair, by name or through a drug class we can link the medicine to, not that a combination is safe. Where our sources name only a group of medicines we cannot match, the cabinet says so. Ask your pharmacist before you combine supplements and medicines.

Who should be careful

6 groups
Kidney problems
If kidney function is reduced, magnesium is cleared slowly and toxicity becomes possible (ODS).1
High doses
Large supplemental doses tend to bring diarrhoea, nausea and cramping; ODS sets 350 mg/day of supplemental magnesium as the upper limit for adults.1
Orotic acid at high intakes
EFSA could not establish the safety of magnesium orotate dihydrate at up to 6,100 mg/day (about 5,000 mg orotic acid), citing animal studies in which orotic acid promoted tumours and noting that the applicant supplied no toxicology data on the product or on orotic acid. That intake also gives about 400 mg of magnesium, above EFSA's upper limit of 250 mg/day for supplemental magnesium.9
Pregnancy and breastfeeding
We found no studies of the orotate form in pregnancy or breastfeeding. Needs rise to 350–400 mg/day in pregnancy and 310–360 mg/day while breastfeeding (all sources); the 350 mg/day upper limit for supplemental magnesium still applies.1
Children
Upper limits for supplemental magnesium are lower: 65 mg/day at ages 1–3 and 110 mg/day at ages 4–8.1
Medicines
See interactions: antibiotic and bisphosphonate timing matters.1

How to read the label

How to read the label

3 checks
Read the elemental number

Because orotate is only about 6.6% magnesium, 1,000 mg of magnesium orotate compound supplies about 66 mg of magnesium. Supplement Facts should list the magnesium amount.21

Label dose vs clinical dose

The heart-failure trial used 3,000 to 6,000 mg of the compound a day, roughly 200 to 390 mg of magnesium, under medical supervision; the upper end exceeds the 350 mg/day supplemental limit.321

Count every source against the upper limit

The 350 mg/day limit covers magnesium from all supplements and medicines combined, such as antacids and laxatives, but not food.1

Common questions

8 answers
What is magnesium orotate good for?

It has been studied mainly for the heart: one 79-patient trial in severe heart failure reported better survival, and Russian trials linked it to fewer rhythm and blood-pressure problems. The evidence is limited.34

Does magnesium orotate help depression?

Only one trial tested it, combined with probiotics and coenzyme Q10, and the benefit faded by follow-up, so no effect can be credited to orotate.5

What is the upper limit for magnesium supplements?

ODS sets 350 mg/day of supplemental magnesium for adults; food magnesium does not count toward it.1

Is magnesium orotate safe?

Trials reported it as generally well tolerated, and large magnesium doses mainly cause diarrhoea and cramping. EFSA (2025) could not establish the safety of very high intakes (up to 6,100 mg/day) because of animal data on orotic acid, and people with reduced kidney function face a risk of magnesium build-up.91

How much magnesium is in magnesium orotate?

About 6.6% by weight for the dihydrate, so 1,000 mg of the compound gives roughly 66 mg of magnesium.2

Does magnesium orotate interact with medicines?

Like other magnesium forms, it can reduce absorption of bisphosphonates and tetracycline or quinolone antibiotics, so timing matters. Diuretics and long-term proton pump inhibitors can change magnesium levels.1

Can I use magnesium orotate while pregnant or breastfeeding?

No studies of the orotate form in pregnancy or breastfeeding were found. ODS sets magnesium needs at 350–400 mg/day in pregnancy and 310–360 mg/day while breastfeeding, with the same 350 mg/day cap on supplemental magnesium.1

Is magnesium orotate better absorbed?

No head-to-head human absorption data were found for orotate versus other forms.1

evidence, doses, interactions, safety, label, FAQA–Z audit. Added two small form-specific trials: ATOMIC post-bypass RCT (PMID 15532307, grade D) and a male-fertility pilot RCT with no effect (PMID 12892384, grade D); studied-dose range updated. Added the 2025 EFSA novel-food opinion on orotic-acid safety (PMID 40642284), pregnancy/breastfeeding and children notes, diuretic and high-dose zinc interactions (all from the NIH ODS magnesium fact sheet, re-read 2026-10-21), a label step and three FAQs. PMIDs checked with esummary and abstracts read with efetch. Fact-checked against the sources; corrected: EFSA cited missing toxicology data from the applicant, not specifically missing human data, and the 250 mg/day limit is EFSA's upper limit (PMID 40642284).Writer: Kymata Health EditorialTrigger: A–Z page audit
