The short answer
From 22 sourcesKudzu root is an isoflavone-rich herb studied mainly for alcohol intake. Small trials from one US group found it cut drinking in heavy drinkers not seeking treatment, but not in two trials of people in alcohol treatment. Menopause results are mixed, and puerarin did not clearly change heart risk factors. Kudzu isoflavones act like estrogen in lab tests.

Key facts
- Drinking, heavy drinkers
- A standardized kudzu extract cut weekly drinks by 34-57% over 4 weeks vs placebo in 17 young men not seeking treatment2
- Single dose
- One 2 g dose (520 mg isoflavones) before a drinking session cut beers consumed from 3.0 to 1.9, while the placebo group rose from 2.7 to 3.4 (n=20)3
- People in treatment
- In 19 adults with alcohol use disorder receiving counseling, kudzu reduced heavy drinking days by 30% vs 44% with placebo over 8 weeks; an earlier VA trial (n=38) also found no benefit56
- Menopause
- A kudzu flower and mandarin peel blend (1,150 mg/day) lowered hot flash scores vs placebo over 12 weeks in 84 women9
- Heart risk factors
- Puerarin 90.2 mg/day did not clearly change lipids or blood pressure in a 217-man crossover trial16
- Liver
- No LiverTox chapter of its own; LiverTox's Chinese herbal medicines chapter cites a review counting 2 causality-confirmed liver injury cases from Pueraria lobata20
What it is
Kudzu is a climbing vine in the pea family whose root, rich in the isoflavone puerarin, is used in Chinese medicine and in supplements.
How it works
ProposedKudzu root contains isoflavones, mainly puerarin, daidzin and daidzein. Trial authors report that kudzu did not make alcohol feel stronger, and it slowed sipping and drinking pace, so the mechanism for reduced drinking is unknown. The isoflavones show estrogen-like activity in laboratory assays.71410

Doses studied in human trials
Doses studied in human trials ranged from 90.2 mg/day of puerarin and 200-300 mg/day of kudzu flower extract to 750 mg/day of root isoflavones for 4-8 weeks, plus a single 2 g dose of extract (520 mg isoflavones); a VA trial used 1.2 g of root extract twice daily (2.4 g/day) and menopause trials used 100 mg/day of isoflavones for 3 months or a 1,150 mg/day flower blend for 12 weeks. Alcohol, for example drinking while using kudzu: After 9 days of kudzu extract, blood alcohol rose slightly faster for 10-15 minutes after a larger alcohol dose (0.7 g/kg), and heart rate and skin temperature effects were stronger; peak alcohol levels and subjective intoxication did not change.

Drugs broken down by CYP2D6 or CYP1A2, for example medicines metabolized by these liver enzymes: In 18 healthy men, oral puerarin lowered CYP2D6 activity and raised CYP1A2 activity (plasma paraxanthine/caffeine ratio up 30%), which could change levels of drugs cleared by these enzymes; Memorial Sloan Kettering notes clinical relevance has not been determined.

Methotrexate
PK studyMeasured in a human study
Methotrexate, for example methotrexate for arthritis, psoriasis or cancer: In rats and cell studies, puerarin raised methotrexate exposure by blocking its intestinal efflux and kidney uptake transporters (MDR1, OAT1/OAT3). No human study exists, so this is an animal and laboratory signal only.

Warfarin and other anticoagulants, for example warfarin (Coumadin): In rats, kudzu root extract sped up warfarin's breakdown in the liver and offset its blood-thinning effect. No human study exists, so this is a laboratory signal only, but a change in warfarin effect matters because its dose is tightly adjusted.

Who should be careful
8 groups- People with alcohol use disorder
- Kudzu did no better than placebo in two small trials of people receiving alcohol treatment, so it is not a substitute for proven care.56
- People with hormone-sensitive conditions
- Kudzu root isoflavones show estrogen-like activity in laboratory assays, and Memorial Sloan Kettering advises people with hormone-sensitive cancers to avoid kudzu. The menopause trials were too short and small to settle safety in breast or uterine conditions.10149
- People with liver problems
- LiverTox has no kudzu chapter. Its Chinese herbal medicines chapter cites a review that counted 2 causality-confirmed liver injury cases from Pueraria lobata, and its Mistletoe chapter describes a man whose liver injury began 10 days after starting a kudzu root extract alongside mistletoe; causality scoring pointed mainly to mistletoe and his tests returned to normal within a week of stopping.202112
- People with kidney problems
- Memorial Sloan Kettering describes a case of acute interstitial nephritis in a woman who drank kudzu root juice for 10 days; symptoms improved within days of stopping.14
- People on methotrexate
- Puerarin raised methotrexate exposure in rats and cell studies. No human study exists, so the size of any effect in people is unknown.11
- People on warfarin or other blood thinners
- In a rat study, kudzu root extract weakened warfarin's blood-thinning effect. There are no human data, so this is a theoretical concern for anyone whose warfarin dose is adjusted by blood tests.15

Frequently asked questions
Short answers to common questions; the numbers link to the sources below.
Common questions
6 answersCan kudzu help someone quit drinking?
Not on current evidence. Kudzu cut drinking in small trials of heavy drinkers not seeking treatment, but in two trials of people in alcohol treatment it did no better than placebo: heavy drinking days fell 30% with kudzu and 44% with placebo over 8 weeks (19 people), and a 38-person VA trial found no difference.256
Does kudzu help hangovers?
Kudzu alone has not been tested for hangovers. Korean trials tested drinks combining Hovenia dulcis with a small amount of kudzu extract, so any effect cannot be credited to kudzu.13
Is kudzu estrogenic?
What are the side effects of kudzu?
Is kudzu safe to take long term?
Is kudzu the same as Pueraria mirifica?
No. Kudzu is Pueraria lobata; Pueraria mirifica is a different Thai species with different compounds and claims.8
