Pharmacologic Augmentation of TMS for Depression With D-serine
Phase 1
Mclean Hospital
About 60 people
Started Apr 1, 2025
In plain words A Phase 1 study of D-Serine in people 18 to 80, run by Mclean Hospital. It plans to enroll about 60 people at 1 US site.
Who it lists and where
Who the trial lists, as registered
Lists people 18 to 80. Conditions: Depression.
Clinical diagnosis of MDD
English-speaking
Adults aged 18-80
Must be able to swallow capsules
3 more inclusion criteria and the exclusion criteria are on the listing.
What it tests, as registered
TMS; D-serine; Placebo
Registered summary
The goal of this study is to test whether combining D-serine with 30 treatments of a) iTBS and b) 18-Hz protocols will enhance clinical outcomes compared to rTMS with placebo (i.e., sugar pill). The investigators hypothesize that NMDA receptor activation via D-serine combined with repeated sessions of rTMS will produce greater clinical outcomes than iTBS with placebo and 18-Hz with placebo.

What it tests, as registered
Modafinil; D-serine solution; Placebo
Registered summary
The goal of this clinical trial is to learn more about decision making in psychosis spectrum disorders, like schizophrenia. Participants will be people who have had symptoms of a psychosis spectrum disorder start within the last five years. The investigators will study how two study agents change decision making in people with psychosis, by asking participants to complete some brain games on the computer before and after taking the study agents. The investigators hope to improve our understanding of psychosis to help people in the future. The full summary is on the listing.

\[¹²³I\]FP-CIT single photon emission CT (DaTscan) confirming dopaminergic nigrostriatal denervation.
Hoehn and Yahr score < 3 at enrollment.
Optimal symptomatic PD treatment, not requiring adjustments, for at least 2 weeks.
1 more inclusion criterion and the exclusion criteria are on the listing.
What it tests, as registered
D-serine; Placebo
Registered summary
This clinical study, designed as a randomized, double-blind, placebo-controlled trial, aims to investigate if modulation of the N-methyl-D-aspartate receptor (NMDAR) via its co-agonist D-serine has therapeutic benefits in Parkinson's disease (PD). All patients will receive both placebo and D-serine over different time periods during the study. Preclinical studies have shown that blocking glycine transporters, which elevates endogenous glycine levels, can restore NMDAR function and improve motor deficits in PD models.
