What the rows share
From the rowsEvery row is graded No effect, our grade for adequately powered trials that found no effect.
The rows run from 598 to 891 people, published from 2016 to 2026: 3 single studies.
The rows
3 no difference- ∅No effectLMTM, a related drug: decline in mild or moderate Alzheimer's disease · No difference
RCT · n 891 · 2016 · People under 90 with mild or moderate Alzheimer's disease at 115 sites in 16 countries
75 or 125 mg twice a day vs 4 mg twice a day as a blinding control · 15 months (65 weeks)
Neither dose differed from control on the co-primary ADAS-cog and ADCS-ADL outcomes. Gastrointestinal and urinary effects were the most common adverse events and the most common reasons for stopping.

- ∅No effectLMTM, a related drug: decline in mild Alzheimer's disease · No difference
RCT · n 800 · 2018 · People with mild Alzheimer's disease
100 mg twice a day vs 4 mg twice a day · 18 months
As randomized, the high dose did not differ from 4 mg twice a day on primary or secondary outcomes (stated in the full text). Before unblinding, the analysis plan was changed to non-randomized comparisons of patients taking LMTM alone, which favored LMTM.

- ∅No effectHMTM, a related drug: cognition and function in early Alzheimer's disease · No difference
RCT · n 598 · 2026 · Amyloid-PET-positive adults with mild cognitive impairment (263) or mild to moderate dementia (335) due to Alzheimer's disease, 82 centers
16 or 8 mg/day vs methylthioninium chloride 4 mg twice weekly as a urine-coloring control · 52 weeks randomized, then all on 16 mg/day to 104 weeks
The co-primary ADAS-cog11 and ADCS-ADL23 outcomes did not separate from control at 52 weeks; the authors attribute this to activity in the control arm. In the mild cognitive impairment group, early vs delayed start differed at 78 and 104 weeks, and blood neurofilament light rose less at 52 weeks.
