Where they differ

From the rows
Study design

The best-graded row, grade B, is a systematic review of 1,253 people (2015) and found an improvement. 2 rows are graded No effect, our grade for adequately powered trials that found no effect.

Only the 2 rows that found an improvement include pooled reviews of several trials (1 of 2); the 2 rows that found no difference are single studies.

2 rows with mixed results are left out of this comparison.

  1. Grade BOsteoarthritis pain (20% reduction, WOMAC) · Improved

Systematic review, 2 studies · n 1,253 · 2015 · Adults with knee osteoarthritis in the 2 low-risk-of-bias trials pooled for this outcome

Varied across trials · 1 month to 3 years across the review

53 of 100 on chondroitin versus 47 of 100 on placebo reached a 20% drop in knee pain (high-quality evidence); across all 43 trials pain scores were 10 points lower in short trials, but with low-quality, heterogeneous evidence.

  2. Grade BKnee osteoarthritis pain and function (pharmaceutical-grade product) · Improved

RCT · n 604 · 2017 · Adults with symptomatic knee osteoarthritis in five European countries

    800 mg/day (celecoxib 200 mg/day as active control) · 6 months (182 days)

Pain on a 100 mm scale fell 42.6 mm with chondroitin, 39.5 mm with celecoxib and 33.3 mm with placebo; chondroitin beat placebo for pain and function (Lequesne index) and did not differ from celecoxib. All treatments were well tolerated.

  3. No effectKnee pain response (20% drop) · No difference

    RCT · n 1,583 · 2006 · Adults with symptomatic knee osteoarthritis (mean age 59)

    1,200 mg/day chondroitin (with or without 1,500 mg/day glucosamine) · 24 weeks

Response was 5.3 points higher than placebo for chondroitin alone (P=0.17) and 6.5 points for the combination (P=0.09), both not significant; celecoxib beat placebo. An exploratory moderate-to-severe pain subgroup did better on the combination.

  4. No effectKnee pain and joint space narrowing over 2 years · No difference

RCT · n 605 · 2015 · Adults aged 45–75 with chronic knee pain and medial joint space narrowing (Australia)

    800 mg/day chondroitin, with or without 1,500 mg/day glucosamine sulfate · 2 years

No group had less knee pain than placebo. Chondroitin alone did not slow joint space narrowing; the combination slowed it by 0.10 mm over 2 years (P=0.046). 6% of participants reported possibly related adverse events.

  5. Grade COsteoarthritis pain (knee or hip) · Mixed

    Meta-analysis, 20 studies · n 3,846 · 2007 · Adults with knee or hip osteoarthritis in 20 trials

Varied across trials · Varied across trials

Heterogeneity was very high (I² 92%); small and lower-quality trials showed larger benefits, and the 3 large intention-to-treat trials showed essentially no effect.

  6. Grade CKnee osteoarthritis pain: pharmaceutical-grade vs other chondroitin · Mixed

    Meta-analysis, 18 studies · n 3,791 · 2019 · Adults with osteoarthritis (mostly knee) in 18 placebo-controlled trials

Mostly 800 mg/day (400–2,000 mg/day across trials, from the full text) · 13 to 104 weeks (from the full text)

Across all trials pain fell (SMD −0.63) and function improved (SMD −0.82), but with extreme heterogeneity (I² 94–95%). In trials at low risk of bias, IBSA's pharmaceutical-grade chondroitin reduced pain (SMD −0.25) while other preparations did not (SMD −0.08, not significant); larger trials showed smaller effects.
