Nopal (Prickly Pear Cactus)

Also called prickly pear cactus, Opuntia ficus-indica, nopales, cactus pear, Indian fig opuntia, nopal cactus extract.

The short answer

From 11 sources

Nopal, the prickly pear cactus pad, is eaten as a vegetable in Mexico and sold as a supplement for blood sugar. Small studies show lower glucose after meals, and a 60-person trial found nopal fiber eased IBS symptoms for a week. A 64-person trial reported fewer severe hangovers. Several trials involved the product's maker, and no agency has reviewed its safety.

Key facts

What it is
Pads (cladodes) of Opuntia cacti, usually Opuntia ficus-indica, traditionally eaten as a vegetable; supplements also use fruit and fruit-skin extracts36
Blood sugar
Small studies of whole nopal or extracts lowered glucose after a meal or glucose drink; a 16-week extract trial found no lasting change567
Gut
20 to 30 g/day of nopal fiber gave more IBS symptom relief than placebo over one week (60 patients)2
Hangover
A crossover trial in 64 young adults reported half the risk of a severe hangover with an Opuntia ficus-indica extract1
Industry ties
Several trials involved the product's maker or seller (Bioserae, Bionap, TriVita)348

What it is

Nopal is the edible pad (cladode) of the prickly pear cactus, mainly Opuntia ficus-indica; supplements use dried pads, fruit or skin extracts.

How it works

Proposed

Nopal pads are a source of soluble and insoluble fiber, which may slow sugar absorption; one study found a low glycaemic index (32.5) and smaller insulin and GIP responses. A cladode and fruit-skin extract raised insulin after a glucose drink in a 6-person study, and an Opuntia extract was followed by lower C-reactive protein after drinking alcohol.1267

Doses studied in human trials

Doses studied in human trials ranged from 200 mg/day of a standardised extract for 16 weeks to 20 to 30 g/day of nopal fiber for 1 week; meal studies added 300 g of steamed pads, and dried-leaf trials used 1.6 g per meal.2356

Starting from one medicine? The interaction checker rates how serious each pair is, on a curated list of 19 entries for 11 medicines. This cabinet has no severity rating: it lists every interaction row our supplement pages cite.

Not medical advice, and no dosing advice. An empty result means no row we cite names that pair, by name or through a drug class we can link the medicine to, not that a combination is safe. Where our sources name only a group of medicines we cannot match, the cabinet says so. Ask your pharmacist before you combine supplements and medicines.

Who should be careful

4 groups
People on diabetes medicines
Nopal lowered glucose after meals in several small studies, and case reports describe low blood sugar when it was combined with diabetes medicines such as glipizide and metformin. NCCIH advises telling your health care providers about any supplement you use.610119
Digestive effects
In the IBS trial, 30 g/day of nopal fiber caused more loose stools than placebo.2
Pregnancy, breastfeeding and children
No trial in our sources studied these groups, and NCCIH notes many supplements have not been tested in pregnant women, nursing mothers or children.9
Agency coverage
No ODS, NCCIH, LactMed, LiverTox, Health Canada or EMA source in our review covers nopal supplements specifically.9
Talk to your pharmacist or clinician before starting a supplement, especially if you take prescription medicines.

Frequently asked questions

Short answers to common questions; the numbers link to the sources below.

Common questions

6 answers
Does nopal lower blood sugar?

After a meal or glucose drink, yes in small studies: 300 g of steamed nopal lowered post-breakfast glucose in 14 people with type 2 diabetes. A 16-week extract trial in prediabetes found no lasting difference from placebo.56

Does prickly pear help hangovers?

One crossover trial in 64 young adults found an Opuntia ficus-indica extract halved the risk of a severe hangover and reduced nausea and dry mouth, though the overall symptom score was not significantly lower.1

Does nopal help IBS?

In a one-week trial of 60 IBS patients, 20 g/day of nopal fiber gave adequate relief to 87% vs 33% on placebo. Longer trials are needed.2

Is nopal safe?

It is a common food in Mexico. In trials, the main side effect was looser stools at 30 g/day of fiber; no agency safety review covers nopal supplements.29

Can nopal be combined with diabetes medicines?

Low blood sugar has been reported when people with type 2 diabetes added nopal to medicines such as glipizide and metformin, including 8 mild cases in one clinic survey. Blood sugar monitoring and a talk with the prescriber matter here.1011

Nopal or prickly pear fruit?

Both come from Opuntia cacti. Pads (nopal) were used in the glucose and IBS studies; an extract and fruit juice were used in the hangover and joint studies.1268
