Hydroxytyrosol

Also called HT, 3,4-dihydroxyphenylethanol, olive polyphenol.

The short answer

From 15 sources

Hydroxytyrosol is the main phenol in olives. The EU's food-safety agency accepted that 5 mg a day of hydroxytyrosol and related phenols in olive oil helps protect blood lipids from oxidation. Supplement trials are small: 15 mg a day lowered oxidised LDL in prediabetes, while a 30-day heart-function trial found no significant change.

Key facts

EU health claim
EFSA accepted a cause-and-effect link with protection of LDL from oxidation at 5 mg/day of hydroxytyrosol and its derivatives in olive oil1
Prediabetes trial
15 mg/day for 16 weeks lowered oxidised LDL versus placebo in 49 completers with prediabetes2
Pooled evidence
14 studies (594 people) of olive phenols: small drops in cholesterol, triglycerides and insulin3
Absorption
Absorbed in a dose-dependent way; plasma levels peaked about 30 minutes after a supplement dose89
Heart function
Mixed: no significant change after 30 days of 10 mg/day in 61 patients with coronary disease; vascular measures improved from baseline in a 30-patient crossover at the same dose514
Blood pressure
An olive extract with 25 mg/day hydroxytyrosol did not lower blood pressure more than placebo over 8 weeks (54 adults)12

What it is

Hydroxytyrosol is a phenolic compound from olives and olive oil, sold as olive-fruit or olive-extract supplements.

How it works

Proposed

Hydroxytyrosol is an antioxidant phenol. After absorption it circulates mainly as conjugates such as hydroxytyrosol-3-O-sulphate and metabolites such as homovanillic acid. The EU claim rests on reduced oxidation of LDL particles; effects on inflammation markers come from small marker trials.128

What the numbers add up to

Each point is worked out from the data and the sources numbered on this page, with the arithmetic shown under it. It adds no new data and is not medical advice.

Markers move, function has not

Oxidative-stress and inflammation markers changed in a 16-week trial, but lipids, weight and physical capacity did not, and a heart-function trial was neutral.25

How we got this: Evidence table on this page

Pooled data mix three compounds

The 2025 meta-analysis pooled oleuropein, hydroxytyrosol and tyrosol, so its small cholesterol and insulin effects are not specific to hydroxytyrosol.3

Studied doses

Doses used in research, shown against the official limits. They describe the studies, not a recommendation for you.

Studied doses in research, not advice. These are amounts used in research, not personal advice. Your clinician or pharmacist can say what fits you.

Doses studied in human trials

Doses studied in human trials ranged from about 5 mg/day of hydroxytyrosol (the low end in a 2024 review of randomised trials, and the low arm of a 24-week weight trial) to 25 mg/day in an olive extract that also supplied 123.5 mg oleuropein, for 8 weeks. 15 mg/day was used for 16 to 24 weeks and 10 mg/day in 1-month coronary-disease trials. Single doses of about 31 and 61 mg were used only to measure absorption. These are research doses, not recommendations.2458121314

Who should be careful

2 groups
Pregnancy, breastfeeding and children
No reliable data. EFSA judged synthetic hydroxytyrosol safe as a food ingredient but left children under 3 years and pregnant and breastfeeding women out of the population it assessed, and LactMed has no hydroxytyrosol entry (checked October 2026). Supplement trials enrolled adults, apart from a 9-patient supervised pilot in children with mitochondrial disease.1110
People with heart disease
Trials in coronary artery disease were small and conflicting (one neutral, one positive mainly versus baseline), so it should not replace prescribed care.514
Talk to your pharmacist or clinician before starting a supplement, especially if you take prescription medicines.

Regulatory status

2 notes

Approved claim in the EU, limited to olive oil.

European Union
EFSA concluded that olive polyphenols standardised to hydroxytyrosol and its derivatives protect LDL particles from oxidative damage at 5 mg/day of hydroxytyrosol and its derivatives in olive oil.1
EU novel food
EFSA concluded that chemically synthesised hydroxytyrosol is safe as a novel food added to fish and vegetable oils (up to 215 mg/kg) and margarines (up to 175 mg/kg), for a general population that excludes children under 3 years and pregnant and breastfeeding women.11

How the body handles hydroxytyrosol

2 notes

Fast absorption, mostly as metabolites.

Peak and metabolites
Plasma levels peaked 30 minutes after a supplement dose; homovanillic acid, hydroxytyrosol-3-O-sulphate and 3,4-dihydroxyphenylacetic acid were the main metabolites in plasma and urine.8
Dose dependence
Absorption rose with dose, and a larger dose increased the share conjugated to glucuronide.89

How to read the label

Hydroxytyrosol or olive polyphenols?

Labels may list total olive polyphenols, hydroxytyrosol, or hydroxytyrosol plus derivatives such as oleuropein and tyrosol. The EU claim counts hydroxytyrosol and its derivatives together.1

Label dose vs studied dose

The EU claim is set at 5 mg/day of hydroxytyrosol and derivatives from olive oil; the supplement trial with a marker benefit used 15 mg/day of hydroxytyrosol.12

Frequently asked questions

Short answers to common questions; the numbers link to the sources below.

Common questions

7 answers
Does hydroxytyrosol lower cholesterol?

Not clearly. A 16-week trial at 15 mg/day did not change the lipid profile; a meta-analysis of olive phenols found small drops in total cholesterol and triglycerides.23

Does it help memory?

One 12-week trial of an olive product found a change in only one attention measure, so the evidence is weak.6

Is hydroxytyrosol safe to take long term?

A 6-month trial at 5 or 15 mg/day found it safe and well tolerated, and a 16-week trial at 15 mg/day saw no adverse events. Longer-term data are lacking, and EFSA's safety assessment excluded pregnant and breastfeeding women and children under 3.13211

How quickly is it absorbed?

In volunteers, plasma levels peaked about 30 minutes after a liquid supplement dose.8

Does hydroxytyrosol interact with medications?

No interactions are documented. LiverTox, LactMed, NCCIH and MedlinePlus have no hydroxytyrosol entry (checked October 2026), and the trials cited here reported none; interactions have not been formally studied.

Is a hydroxytyrosol supplement better than olive oil?

No trial has compared them on health outcomes. The EU claim covers olive oil supplying 5 mg/day of hydroxytyrosol and its derivatives, while capsule trials used 5–25 mg/day of hydroxytyrosol with mixed results on markers, blood pressure and heart function.1212

What does the EU health claim say?

That olive oil polyphenols help protect blood lipids from oxidative stress, at 5 mg/day of hydroxytyrosol and its derivatives in olive oil.1
