Citicoline vs Alpha-GPC: Two Choline Supplements, Very Different Evidence

Both are sold for focus and memory, and both supply choline. The direct comparisons come from injection studies and dementia trials. Citicoline's best healthy-adult data is for memory in older adults; alpha-GPC's is for short-term strength and attention tests in young men. Here is what each trial found and the safety signal worth knowing.

Quick pick

Jarrow Formulas Citicoline (Cognizin), 250 mg, 60 capsules

from HerbsPro

Its name matches citicoline sodium (e.g. Cognizin), one of the forms compared in our Forms table.

Label: 1 capsule: 250 mg citicoline (Cognizin).

Best for trying one bottle: $24.99 one-time (checked Oct 12, 2026).

How we chose: Momentous is the partner brand we list first, so its product leads whenever the Where to buy list has one for this compound or a form in our Forms table. Otherwise, among partner brands that passed our vetting without conditions (others only when none sells it), the first product in that list whose name matches a form in our Forms table, else the first one listed. Not a claim that it works: check the amount per serving against the studied doses and limits on our Citicoline page.

Key takeaways

From 11 sources
  • Direct comparisons exist only in dementia or by injection: a 2025 meta-analysis of 3 trials (358 people with dementia) favored alpha-GPC on a global clinical scale but found no difference on memory tests11, and by injection, 1,000 mg of alpha-GPC raised blood choline more than 1,000 mg of citicoline3.
  • Citicoline at 500 mg a day for 12 weeks improved episodic and composite memory scores (secondary outcomes) more than placebo in 100 healthy adults aged 50 to 85 with age-associated memory impairment5.
  • Alpha-GPC at 600 mg a day for 6 days raised lower-body force in 13 college-aged men7, and a single 315 or 630 mg dose improved one attention test in 20 trained men8.
  • In more than 12 million Korean adults aged 50 or older, people prescribed alpha-GPC had a higher 10-year stroke risk (adjusted hazard ratio 1.43 after matching); the study was observational and cannot show cause10.
  • Total choline from food and supplements has an adult upper limit of 3,500 mg a day; very high intakes are linked to fishy body odor and low blood pressure1.

Two routes to the same nutrient

Choline is needed to make acetylcholine, a neurotransmitter involved in memory, mood and muscle control1. Citicoline (CDP-choline) and alpha-GPC (choline alfoscerate) are both choline-containing compounds sold as acetylcholine precursors2.

For blood choline, the form may matter less than expected. In a crossover study, 6 men took 550 mg of choline as choline chloride, choline bitartrate, alpha-GPC or egg phosphatidylcholine, and blood choline exposure did not differ between forms9. The water-soluble forms, alpha-GPC included, quickly raised TMAO, a gut-bacteria metabolite, while egg phosphatidylcholine did not9. Citicoline was not tested9. For the cheaper form, see alpha-GPC vs choline bitartrate.

The head-to-head studies: injections and dementia trials

In a 1992 crossover, 12 healthy volunteers received 1,000 mg of alpha-GPC and 1,000 mg of citicoline by intramuscular injection on separate days3. Blood choline rose quickly after both, but levels were considerably lower after citicoline3.

A 1991 open-label trial gave 120 people with mild to moderate vascular dementia 1 g a day of either compound by injection for 90 days4. Both groups improved and both were well tolerated, and alpha-GPC scored statistically better on most tests4. A 2025 meta-analysis pooled 3 randomized trials with 358 people with dementia: alpha-GPC scored better than citicoline on a global clinical scale (SCAG), but there was no significant difference on memory or word-fluency tests, and dropout rates were similar (9.4% vs 6.7%)11. All of this comes from injection studies or dementia populations, so none of it shows how capsules compare in healthy adults3411.

Citicoline: memory in older adults, attention in teens

In a 12-week trial in 100 healthy men and women aged 50 to 85 with age-associated memory impairment, 500 mg a day of citicoline (Cognizin) improved episodic memory and a composite memory score more than placebo; these were secondary outcomes5. Authors included employees of Kyowa Hakko Bio5.

In 75 healthy adolescent boys, 250 or 500 mg a day for 28 days improved attention and psychomotor speed compared with placebo6. Kyowa Hakko staff were also among this trial's authors6. Our search found no exercise-performance trials of citicoline. The citicoline page has the full evidence table.

Alpha-GPC: short-term force and attention in young men

In a crossover trial, 13 college-aged men took 600 mg a day of alpha-GPC or placebo for 6 days7. Peak force on an isometric mid-thigh pull rose more with alpha-GPC (98.8 N versus a 39.0 N drop with placebo), while the upper-body test did not reach significance7.

In 20 resistance-trained men, single doses of 630 mg or 315 mg improved Stroop test scores 60 minutes later compared with placebo, but not the Flanker or N-Back tests, and physical performance did not differ8. The author is a paid scientific advisor to the study's sponsor, NNB Nutrition8. Alpha-GPC's trials in older adults were in people with cognitive impairment or dementia; see the alpha-GPC page.

The safety signal worth knowing

A South Korean national cohort followed 12,008,977 adults aged 50 or older for stroke over roughly the next decade10. The 108,877 people prescribed alpha-GPC had higher risks of total stroke (adjusted hazard ratio 1.43 after matching), ischemic stroke and hemorrhagic stroke, with risk rising as use increased10. As an observational study it cannot rule out differences between users and nonusers that the adjustments missed, and the authors called for studies of mechanism10.

ODS notes that choline intake raises TMAO in a dose-dependent way, and TMAO has been linked to higher cardiovascular risk1. Adults have an Adequate Intake of 425 mg a day (women) and 550 mg (men), and an upper limit of 3,500 mg a day from all sources; high intakes are associated with fishy body odor, sweating, vomiting and low blood pressure1.

Which one fits your goal

What the trials above support, by goal:

  • Memory in midlife and later: citicoline has the healthy-adult trial, at 500 mg a day for 12 weeks5.
  • Pre-workout power or focus: alpha-GPC has small short-term trials at 315 to 630 mg78.
  • Raising blood choline: equal choline doses of alpha-GPC and other forms gave similar blood choline in one small study9.
  • Stroke risk factors or heart concerns: the observational stroke signal for alpha-GPC is worth discussing with a clinician10.
  • Stacking choline products: count total choline toward the 3,500 mg adult upper limit1.

Quick answers

Common questions

3 answers
Can you take citicoline and alpha-GPC together?

We found no trial testing the two together. If you combine them, total choline from all sources counts toward the adult upper limit of 3,500 mg a day1.

Which is better before a workout?

Alpha-GPC is the one with workout trials: 600 mg a day for 6 days raised lower-body force7, and a single dose improved a Stroop test but not physical performance8.

Does alpha-GPC cause strokes?

That has not been shown. A large Korean cohort found people prescribed alpha-GPC had a higher 10-year stroke risk, but the design cannot prove cause10.
