Berberine vs Inositol for PCOS: More Trials on One Side, Integrity Questions on the Other
Inositol has the larger PCOS trial base, though the review behind the 2023 international guideline still called it inconclusive. Berberine's PCOS trials look promising on paper, but an independent integrity review could not trust any of the 4 it assessed.

Key takeaways
From 11 sources- In 26 trials with 1,691 women with PCOS, regular menstrual cycles were 1.79 times as likely with inositol as with placebo1.
- The meta-analysis done for the 2023 international PCOS guideline still called the evidence for inositol limited and inconclusive2.
- A 12-trial berberine meta-analysis found lower testosterone than placebo or no treatment, but no solid evidence of better live birth rates or other clinical outcomes4.
- A 2026 integrity review rated all 4 eligible berberine PCOS trials, with 452 women, at high risk of compromised integrity and concluded that no reliable conclusions can be drawn5.
- In a 2-week study, berberine reduced the activity of 3 drug-metabolising enzymes, raising exposure to the test drug midazolam by 40%8.

Why these two get compared
Insulin resistance is common in PCOS, and both inositol and berberine are studied as insulin-sensitising options alongside the prescription drug metformin26. A 2021 network meta-analysis of 22 trials with 1,079 women compared metformin, thiazolidinediones, inositol and berberine for PCOS in a single analysis6.
The 2023 international PCOS guideline continues to emphasise healthy lifestyle, emotional wellbeing and quality of life, and rates the evidence in the field as generally low to moderate quality3.
Inositol: more trials, still uncertain
A 2023 meta-analysis of 26 trials included 1,691 women with PCOS: 806 on inositol, 311 on placebo and 509 on metformin1. Regular cycles were 1.79 times as likely as with placebo (95% CI 1.13 to 2.85), and inositol was non-inferior to metformin on this outcome1. BMI, free and total testosterone, androstenedione and glucose fell more than with placebo, and SHBG rose1.
The review commissioned for the 2023 international guideline was more cautious. Across 30 trials with 2,230 women, it found benefits for some metabolic measures and possible ovulation benefits from D-chiro-inositol, but possibly no effect on other outcomes2. Metformin may improve waist-hip ratio and excess hair growth more than inositol, while myo-inositol likely causes fewer stomach side effects than metformin2. Its authors called the evidence limited and inconclusive2.
In the network meta-analysis, myo-inositol combined with D-chiro-inositol improved cycle frequency more than metformin, though the estimate was very imprecise (odds ratio 14.70, 95% CI 2.31 to 93.58)6. For more, see what the inositol trials show and inositol vs metformin.
Berberine: promising numbers, trials that could not be trusted
A 2019 meta-analysis of 12 trials, which searched Chinese as well as English databases, found berberine lowered total testosterone and the LH to FSH ratio compared with placebo or no treatment4. Live birth rates were similar to placebo or metformin and lower than with the fertility drug letrozole (RR 0.61)4. The authors found no solid evidence that berberine improves live birth or other clinical outcomes4.
A 2026 review then checked the trials themselves. All 4 trials that met its criteria, with 452 women, were rated at high risk of compromised integrity, with concerns including no trial registration, implausible recruitment timelines and possible duplicated data5. The reviewers concluded that reliable conclusions about berberine in PCOS cannot be drawn from the current evidence5.
Berberine's larger evidence base is outside PCOS. In 14 placebo-controlled trials with 1,447 adults, mostly in mainland China and Hong Kong, it lowered LDL cholesterol by 0.46 mmol/L10. See the berberine page and is berberine nature's Ozempic? for that evidence.
The one head-to-head trial
A 2022 randomised trial gave women with PCOS berberine 500 mg, metformin 500 mg or myo-inositol 1,000 mg, each twice a day, for 3 months7. The authors reported that berberine produced larger changes in body measures, hormones and blood lipids, while myo-inositol produced larger improvements in glucose and insulin measures7.
The abstract does not report group sizes or between-group effect sizes, and a single trial cannot settle the question7.
Safety and interactions
After 2 weeks of berberine at 300 mg three times a day, healthy men showed lower activity of the drug-metabolising enzymes CYP2D6, CYP2C9 and CYP3A4, and exposure to the test drug midazolam rose 40%8. Goldenseal, a herb that contains berberine, lowered metformin levels by about 25% in healthy adults in an NCCIH-funded study9.
NCCIH advises that people who are pregnant or breastfeeding should not use goldenseal, and that its berberine constituent can be harmful to newborns9. In the 12-trial PCOS review, stomach side effects were no more common than with placebo4, while in lipid trials they tended to be more frequent with berberine (2 to 23% vs 2 to 15%)10.
For inositol, a review of the safety data found that only the highest dose, 12 g a day, caused mild nausea, gas and diarrhoea11.
Which one fits you
- Irregular cycles in PCOS: inositol has more trials, though the guideline review called them inconclusive12.
- High cholesterol or triglycerides: berberine's lipid evidence in adults generally is stronger than its PCOS-specific evidence510.
- Already on metformin or other regular medicines: check with a pharmacist before adding berberine, because of the enzyme and metformin findings above89.
- Pregnant or breastfeeding: avoid berberine-containing goldenseal, per NCCIH9.
