Alanylglutamine

Also called L-alanyl-L-glutamine, Ala-Gln, Sustamine (brand), alanyl-glutamine dipeptide.

The short answer

From 16 sources

Alanylglutamine is a two-amino-acid dipeptide that delivers glutamine more efficiently than free glutamine: plasma glutamine exposure was about twice as high at the same glutamine dose. Small sports studies suggest it may help endurance under dehydration. In undernourished children, 10 days at 3–24 g/day improved gut-permeability tests and weight gain.

Key facts

Absorption
Plasma glutamine area under the curve 284 vs 127 µmol·h/L for the dipeptide vs free glutamine at equal glutamine doses3
Endurance
Longer time to exhaustion under mild dehydration at 0.05–0.2 g/kg in 10 men1
Children's gut trials
3–24 g/day for 10 days improved gut-permeability tests and weight gain in two Brazilian trials56
Critical illness
Glutamine given by vein (as the alanyl-glutamine dipeptide) and by tube was linked to higher in-hospital and 6-month mortality in a 1,223-patient ICU trial79

What it is

Alanylglutamine is a dipeptide of alanine and glutamine, used in sports drinks and clinical nutrition.

How it works

Proposed

The dipeptide is absorbed intact and split to release glutamine, giving a larger and longer plasma glutamine rise than free glutamine. In rats after resistance exercise it reduced signalling linked to muscle protein breakdown but did not boost synthesis signalling.38

Each point is worked out from the data and the sources numbered on this page, with the arithmetic shown under it. It adds no new data and is not medical advice.

Most sports evidence compares against no drink

The endurance benefit was measured against no rehydration; against plain water or electrolyte drinks, differences were small or unclear.14

Studied doses in research, not advice. These are amounts used in research, not personal advice. Your clinician or pharmacist can say what fits you.

Doses studied in human trials

Doses studied in human trials ranged from 300 mg to 1 g per 500 mL of sports drink, or 0.05–0.2 g/kg, in single exercise sessions, to 3–24 g/day for 10 days in undernourished children and adults with HIV, and 8 g/day (with fish oil) for 3 months in heart failure. Intravenous hospital use (0.3–0.5 g/kg/day) is clinical nutrition. These are research doses, not recommendations.1256111213

Starting from one medicine? The interaction checker rates how serious each pair is, on a curated list of 19 entries for 11 medicines. This cabinet has no severity rating: it lists every interaction row our supplement pages cite.

Not medical advice, and no dosing advice. An empty result means no row we cite names that pair, by name or through a drug class we can link the medicine to, not that a combination is safe. Where our sources name only a group of medicines we cannot match, the cabinet says so. Ask your pharmacist before you combine supplements and medicines.

Who should be careful

4 groups
Critically ill people
In a trial of 1,223 ventilated ICU patients with multiorgan failure, high-dose glutamine given by vein (as the alanyl-glutamine dipeptide) and by tube (as alanyl-glutamine and glycine-glutamine dipeptides) was linked to higher in-hospital and 6-month mortality.79
Children
Paediatric data come from supervised trials in undernourished children, not from routine supplement use.56
Pregnancy and breastfeeding
No alanylglutamine trial enrolled pregnant or breastfeeding women, and LactMed has no entry for it. NCCIH notes that many dietary supplements have not been tested in pregnant women, nursing mothers or children.10
Liver disease
Alanylglutamine releases glutamine. LiverTox rates L-glutamine likelihood score E (unlikely cause of acute liver injury with jaundice), but notes that 10 to 20 g of glutamine raised blood ammonia and worsened hepatic encephalopathy measures in people with decompensated cirrhosis, so it should be avoided in advanced cirrhosis.16
Talk to your pharmacist or clinician before starting a supplement, especially if you take prescription medicines.

Safety notes

2 notes

Short trials, one serious hospital signal.

Intensive care
Early glutamine plus antioxidants did not improve outcomes, and glutamine, given largely as alanyl-glutamine, was associated with higher mortality in critically ill adults with multiorgan failure.79
Tolerance
Children tolerated alanyl-glutamine formula well in a 10-day trial.6

Frequently asked questions

Short answers to common questions; the numbers link to the sources below.

Common questions

6 answers
Does it improve endurance?

In 10 men under mild dehydration, it lengthened time to exhaustion compared with no rehydration.1

Does it help hydration?

A dipeptide-electrolyte drink retained more fluid than water at 4 hours but no more than electrolytes alone.4

Does alanylglutamine help after surgery or in intensive care?

Only as hospital nutrition by vein. A 2015 meta-analysis of 19 surgery trials (1,243 patients) found 2.67 fewer hospital days but no drop in complications; a 2017 industry-authored meta-analysis of ICU trials found fewer infections and deaths, but the 1,223-patient REDOXS trial linked high-dose glutamine to higher mortality.14137

Is it safe?

Short trials report good tolerance, but in critically ill ICU patients high-dose glutamine, given largely as alanyl-glutamine, was linked to higher mortality.1679

Is it better absorbed than glutamine?

In 8 men it produced about twice the plasma glutamine exposure of free glutamine at the same glutamine dose.3

What is alanylglutamine?

A dipeptide made of alanine and glutamine, used in sports drinks and clinical nutrition.3
