Fenugreek and Blood Sugar: Lower Readings in Pooled Trials, Low-Quality Evidence
Two 2023 meta-analyses found lower blood-sugar markers with fenugreek, but they disagree on how much, and NCCIH says many of the studies were not of high quality.
Key takeaways
- A 2023 meta-analysis of 10 trials (706 participants) in adults with type 2 diabetes or prediabetes found lower fasting glucose, 2-hour glucose and HbA1c with fenugreek [1].
- Insulin resistance (HOMA-IR) did not change significantly in that analysis [1].
- A second 2023 meta-analysis of 14 trials (894 participants) found only HbA1c significantly lower and named study quality and heterogeneity as concerns [2].
- NCCIH says research suggests fenugreek may help lower blood sugar, but many of the studies were not of high quality [3].
- NCCIH also warns that large doses may cause a harmful drop in blood sugar [3].
The evidence, graded
Pulled from our monograph evidence tables. Grades follow our evidence-grading policy.
- Grade CWorsenedFasting and 2-hour blood sugar and HbA1cFasting blood sugar, 2-hour glucose and HbA1c fell versus control; insulin resistance (HOMA-IR) did not change significantly. No liver or kidney toxicity or severe adverse events; mild digestive side effects in some studies.
- Grade CWorsenedBlood lipids (total and LDL cholesterol, triglycerides, HDL)Total cholesterol fell by about 9.4 mg/dL, triglycerides by 13.8 mg/dL and LDL by 6.6 mg/dL, and HDL rose by 3.5 mg/dL versus control. Body weight and BMI did not change.
What the pooled trials found
A 2023 meta-analysis pooled 10 randomized trials with 706 participants who had type 2 diabetes or prediabetes [1]. Fasting blood glucose, 2-hour glucose on an oral glucose tolerance test and HbA1c fell significantly versus control, while HOMA-IR, a measure of insulin resistance, did not [1].
A second 2023 meta-analysis of 14 trials with 894 participants found a statistically significant difference only for HbA1c (mean difference −0.88) [2]. Its confidence intervals for fasting and after-meal glucose crossed zero [2].
Neither abstract gives a single dose, form or trial length, so the pooled results cannot be tied to one product [1][2].
Why the evidence is graded low
NCCIH says there is not enough high-quality evidence to determine whether fenugreek is useful for diabetes, and that many of the blood-sugar studies were not of high quality [3]. The second meta-analysis likewise calls study quality and heterogeneity a concern and asks for rigorous double-blind trials [2].

Blood lipids
A 2020 meta-analysis of 12 trials (14 arms, 560 adults) found total cholesterol about 9.4 mg/dL lower, triglycerides about 13.8 mg/dL lower, LDL about 6.6 mg/dL lower and HDL about 3.5 mg/dL higher with fenugreek; body weight and BMI did not change [4].
The 2023 analysis in people with high blood sugar also found better total cholesterol, triglycerides and HDL, but no significant difference in LDL [1].
Safety notes on blood sugar
NCCIH warns that large doses may cause a harmful drop in blood sugar, and advises anyone taking any medicine to talk with a health care provider before using fenugreek [3]. That warning matters most for people on blood-sugar-lowering medicines.
NCCIH says fenugreek is not safe during pregnancy in amounts greater than those found in food [3]. In the 10-trial meta-analysis, no liver or kidney toxicity or severe adverse events were seen, though some studies reported mild digestive side effects [1].

Quick answers
Does fenugreek tea or seed lower blood sugar differently from capsules?
Our sources do not separate teas, seeds and extracts; the pooled abstracts do not break results down by form [1][2].
How long did the blood-sugar trials last?
The pooled abstracts do not report trial lengths, so our sources cannot say how quickly changes appeared [1][2].
Is fenugreek safe with blood-sugar medicines?
NCCIH warns that large doses may cause a harmful drop in blood sugar and advises talking with a health care provider before use if you take any medicine [3].
